| 目的 应用动态MRI探讨分娩方式对女性盆膈裂孔大小的影响.方法 对51名不同分娩方式的产后女性行盆底动态MR扫描,分别测量静息状态及最大用力状态下盆膈裂孔前后径、左右径及面积,比较不同分娩方式产后女性盆膈裂孔大小的差异.结果 MR轴位图像可清晰显示盆膈裂孔及其内的尿道、阴道、直肠等盆底结构;静息状态下盆膈裂孔呈菱形,最大用力状态下肛提肌下移呈椭圆形.静息状态及最大用力状态下,经阴道顺产与剖宫产者的盆膈裂孔大小差异无统计学意义(P>0.05),均小于经阴道低位产钳助产者(P均<0.05).结论 应用动态MRI可以清晰显示盆膈裂孔,并准确测量其大小;分娩方式是盆膈裂孔大小的影响因素之一. |

| Objective To investigate the impact of different delivery modes on female pelvic diaphragm hiatus with dynamic MRI. Methods Totally 51 postpartum females with different delivery modes underwent dynamic MRI scan of pelvic floor. The diameter, transverse diameter and area of pelvic diaphragm hiatus in resting and Valsalva state were measured and compared among different delivery modes. Results The pelvic diaphragm hiatus and the pelvic structure, such as urethra, vagina and rectum were clearly displayed on axial images. The pelvic diaphragm hiatus was rhombus in resting state, while became oval along with the anal levator moving down in Valsalva state. No significant difference was found in the size of pelvic hiatus between spontaneous delivery group and uterine-incision delivery group (P>0.05), while vaginal forceps delivery group had larger pelvic diaphragm hiatus than the other two groups (both P<0.05) both in resting and Valsalva state. Conclusion The pelvic diaphragm hiatus can be displayed clearly on dynamic MRI, and the size of it can also be measured exactly. Delivery mode is one of the impact factors on the size of pelvic diaphragm hiatus. |
