| 目的 分析声像图表现为子宫下段混合回声包块的剖宫产瘢痕妊娠(CSP)的临床及声像图特点。方法 回顾分析声像图表现以子宫前壁下段剖宫产瘢痕处混合回声包块为特点的CSP患者52例,分析其病史、二维及CDFI特点。将其中接受手术治疗(包括清宫术)的40例按出血量分为<200 ml组(26例)和≥200 ml组(14例),比较两组间的临床及声像图特征。结果 本组病例临床表现包括停经、阴道不规则出血以及人工流产或药物流产后HCG下降不满意或反而升高,14例曾出现大出血。病灶声像图特点为位于子宫前壁下段的混合回声包块,血流分布以周边为主。治疗出血量<200 ml组和≥200 ml组之间在停经时间、病变最大径、平均径和阻力指数(RI)值差异均有统计学意义。结论 包块型CSP的声像图具有较特征性表现,可为临床决策提供依据。 |

| Objective To explore the clinical and ultrasonographic features of cesarean scar pregnancy (CSP) shown as lower uterine segment mass. Methods Fifty-two CSP patients shown as lower uterine segment mass in ultrasound were enrolled, and the clinical history, gray scale and CDFI appearances were analyzed. Forty patients received operations, among which 26 with hemorrhage amount <200 ml (<200 ml groups) and 14 patients with hemorrhage amount ≥200 ml (≥200 ml group). The clinical and ultrasonographic features of the two groups were compared. Results The main clinical appearances included menopause, irregular vaginal bleeding and abnormal serum β-HCG level after abortion. Fourteen patients experienced heavy bleeding before coming to our hospital. The ultrasound showed heterogeneous mass in the anterior lower uterine segment with various level of surrounding blood signal. The menopause time length, maximum size, average size of lesion and resistance index (RI) value showed significant differences between operation hemorrhage amount <200 ml and ≥200 ml groups. Conclusion The patients with CSP shown as heterogeneous mass have certain clinical and ultrasonic features, and the latter may provide helpful information for clinical treatment option. |
