| 目的 分析多微脑回畸形(PMG)的MRI特征。方法 对39例PMG的MRI表现进行回顾性分析。结果 39例中,PMG位于外侧裂周围脑皮质23例,额叶3例,额顶叶6例,顶枕叶4例,双侧额、顶、枕叶弥漫受累2例,左颞叶1例;表现为大脑皮质增厚、脑沟变浅的粗糙型32例,大脑皮质厚度正常或变薄,脑回、脑沟变小的纤细型7例;19例外侧裂延长或增宽,14例局部脑皮质内折、切迹,12例PMG下方脑白质出现T2WI高信号;双侧病变14例,单侧病变25例。结论 PMG的MRI表现具有一定特征性;MRI可以清晰显示PMG的病变部位、范围及类型。PMG的临床表现及预后与病变范围密切相关,双侧病变者临床表现严重程度大于单侧病变。 |

| Objective To analyze MRI appearances of polymicrogyria (PMG). Methods MRI appearances of 39 patients with PMG were analyzed retrospectively. Results Among 39 patients, the lesions located at the cortex surrounding perisylvian in 23, at frontal in 3, at frontoparietal in 6, at parietooccipital in 4, at bilateral frontal-parieto-occipital in 2 and left temporal in 1. Coarse type PMG with thick cortex was detected in 32 patients, while delicate type PMG with normal or thin cortex was detected in 7 patients. The sylvian fissures abnormally extended posteriorly into the parietal lobe or broadened in 19 patients. Infolding of abnormal cortex was found in 14 patients. Twelve patients showed hyperintensity in underlying white matter on T2WI. Fourteen patients had bilateral, whereas 25 had unilateral involvement. Conclusion MRI features of PMG have some characteristics. MRI can provide information of the site, extent and morphologies of PMG. The clinical presentations of patients with bilateral PMG involvement are more severe than those of unilateral involvement. |
