      目的 探讨CEUS评价兔胶原诱导性关节炎(AIA)模型膝关节滑膜炎的可行性及应用价值。方法 选取30只新西兰大白兔均分为A、B组,分别将2 mg及8 mg鸡卵蛋白注入兔右侧膝关节腔制成AIA模型。于4周后行二维超声、CEUS及增强MR检查,评估滑膜炎严重程度,将超声与MRI结果进行对比,并行相关性分析。结果 二维超声及MR均显示A、B组兔AIA模型膝关节滑膜炎的发生率及分级差异有统计学意义(P均<0.05)。CEUS与增强MRI对于兔AIA模型滑膜炎的检出率及评价滑膜炎分级的差异均无统计学意义(P>0.05),二者对于滑膜炎分级的评价一致性较高(Kappa=0.78,P<0.05),相关性高(r=0.94,P<0.05)。以MRI作为金标准,二维超声诊断兔AIA模型膝关节滑膜炎的敏感度为72.73%(16/22),特异度为97.37%(37/38),准确率为88.33%(53/60),Youden指数为0.70,CEUS 的敏感度为90.91%(20/22),特异度为100%(38/38),准确率为96.67%(58/60),Youden指数为0.91。结论 CEUS可作为评价AIA兔模型滑膜炎严重程度及其治疗性实验效果随访的新方法。

      Objective To explore the feasibility and value of CEUS in evaluating synovitis of antigen-induced arthritis (AIA) rabbit models. Methods Thirty New Zealand white rabbits were equally divided into group A and group B, and were injected 2 mg and 8 mg ovalbumin, respectively. At the 4th week, two-dimensional ultrasound, CEUS and enhanced MRI were performed to evaluate the severity degree of synovitis. These results of US and MR were compared, and the correlation was analyzed. Results The incidence rate of synovitis and score between group A and group B by both US and MRI were statistically different (both P<0.05). The difference of synovitis detection rate and score by CEUS and enhanced MRI had no statistical significance (P>0.05). The consistency (Kappa=0.78, P<0.05) and correlation (r=0.94, P<0.05) for synovitis score between CEUS and MRI were good. Taking the results of MRI as golden standards, the sensitivity, specificity, accuracy rate and Youden index of US and CEUS in the diagnosis of synovitis in AIA model was 72.73% (16/22), 97.37% (37/38), 88.33% (53/60), 0.70 and 90.91% (20/22), 100% (38/38), 96.67% (58/60), 0.91, respectively. Conclusion CEUS can be applied as a new strategy for evaluation of synovitis and following up of the therapy effect in AIA rabbit models.
