目的 评价宝石CT能谱成像鉴别肝脏小囊肿与微小转移瘤的价值。方法 对26例患者在3~6个月中行2~3次CT能谱成像双期扫描,共获得40个直径≤1cm病灶,包括23个肝脏微小转移瘤、17个肝脏小囊肿。在门静脉期选择相同大小的ROI,测定两种病灶的能谱曲线及斜率、碘-水浓度和有效原子序数,并分别行独立样本t检验。结果 肝脏微小转移瘤的能谱曲线均呈速降型,平均斜率为0.91±0.27;16个肝脏小囊肿的曲线呈缓慢下降型,平均斜率为0.43±0.14,1个小囊肿的曲线呈勺状,斜率为0.54;肝脏微小转移瘤的能谱曲线斜率明显大于肝脏小囊肿(t=6.831,P<0.001)。肝脏微小转移瘤的平均碘(水)浓度、水(碘)浓度分别为(10.83±3.18)g/L、(1030.24±11.72)g/L,有效原子序数为8.26±0.19;肝脏小囊肿的平均碘(水)浓度、水(碘)浓度分别为(5.04±1.68)g/L、(1005.96±5.79)g/L,有效原子序数为7.89±0.11,均小于肝脏微小转移瘤(t=7.428、8.611、7.357,P均<0.001)。结论 肝脏微小转移瘤与肝脏小囊肿具有不同的能谱曲线及能谱特征性物质含量。能谱CT能够有效地鉴别二者。 |

Objective To assess the utility of gemstone spectral CT imaging in differentiating small hepatic cysts from micro-metastatic tumors. Methods Twenty-six patients underwent gemstone spectral imaging contrast enhanced examination of upper abdominal twice or three times within 3—6 months. Totally 40 lesions with diameter ≤1 cm had been displayed, including 23 micro-metastatic lesions and 17 small hepatic cysts. Spectral curves, slope rates, iodine-water concentration and effective-Z were measured with the same size ROI on the lesions on portal venous phase images. The differences of these spectral characteristic parameters were evaluated statistically by independent samples t test. Results All micro-metastatic lesions displayed fast-down curves, and the mean slop rate was 0.91±0.27. Among small hepatic cysts, 16 showed slow-down curves with a mean slop rate of 0.43±0.14, and 1 displayed slow-up curve with a slop rate of 0.54. The mean slope rate of micro-metastatic lesions was significantly larger than that of hepatic cysts (t=6.831, P<0.001). The mean iodine-water concentration and water-iodine concentration ([10.83±3.18]g/L,[1030.24±11.72]g/L) and effective-Z (8.26±0.19) of micro-metastatic lesions were significantly higher than those of small hepatic cysts ([5.04±1.68]g/L, [1005.96±5.79]g/L, [7.89±0.11]; t=7.428, 8.611, 7.357, all P<0.001). Conclusion The spectral curve and spectral imaging parameters of micro-metastatic lesions are different from those of small hepatic cysts. Gemstone spectral CT imaging provides an effective method for differentiating micro-metastatic lesions from small hepatic cysts. |
