      目的 探讨乳腺癌的彩色多普勒超声表现与肿瘤大小、组织病理学类型的相关性。方法 回顾性分析200例乳腺癌患者202个肿瘤的彩色多普勒超声表现,分析乳腺癌血流分布类型、血流分级与其大小、组织病理学类型的相关性。结果 202个乳腺癌病灶的血流检出率为91.58%(185/202),周边型血流17.33%(35/202),内部型血流51.49%(104/202),穿入型血流22.77%(46/202),8.42%(17/202)无血流信号。非浸润性乳腺癌、浸润性导管癌、特殊类型乳腺癌之间及浸润性导管癌Ⅰ级、Ⅱ级、Ⅲ级之间血流分布类型、Adler分级的差异均无统计学意义(P均>0.05);但不同大小乳腺癌之间血流分布类型、Adler分级差异有统计学意义,肿块越大,内部型血流、穿入型血流越多,Adler分级越高(P<0.05)。结论 乳腺癌血流分布类型、血流分级与大小均呈正相关。乳腺癌彩色多普勒超声表现与其组织病理学类型无明显相关性。
英文摘要:
      Objective To explore the correlation between color Doppler ultrasound manifestations and the size and histopathology of breast cancer. Methods Color Doppler ultrasound findings, including vascular patterns and blood flow classifications of 202 lesions in 200 patients of breast cancer were retrospectively reviewed. The correlation between color Doppler ultrasound manifestations and the size and histopathology of breast cancer were analyzed. Results In 202 breast cancers, the total detection rate for blood flow was 91.58% (185/202). Peripheral vascularity was detected in 17.33% (35/202), central vascularity in 51.49% (104/202), while penetrating vascular in 22.77% (46/202), avascular in 8.42% (17/202). The blood flow distribution type and Adler grade had no statistical difference (P>0.05) among non-invasive carcinomas, invasive ductal carcinoma and specified invasive carcinoma, nor among invasive ductal carcinoma gradeⅠ, gradeⅡ and grade Ⅲ, but statistical differences were found among different tumor sizes (P<0.05). Adler grade increased along with the size of tumor. The bigger the mass was, the more central vascularities and penetrating vascularities, and the higher Adler grade. Conclusion The size of breast cancer showed positive correlation both with its blood flow distribution and blood flow classification, while no correlation between color Doppler ultrasound manifestations and histopathology of breast cancer was found.
