目的 采用超声断层显像(TUI)技术评估女性盆腔器官脱垂(POP)与肛提肌-尿道间隙(LUG)的关系,探讨POP患者肛提肌形态学的特征。方法 三维超声扫查54例POP患者(POP组)及58例妇科良性疾病患者(对照组),分别在静息和Valsalva状态下获取容积影像数据;运用QLab处理软件以2.5 mm层间距获取三层轴位图像,分别测量LUG值,并观察肛提肌的形态。结果 静息状态下及Valsalva状态下POP组的双侧LUG均值均大于对照组[(2.76±0.56)cm vs (2.08±0.37)cm,P<0.001;(3.04±0.86)cm vs (2.24±0.54)cm,P<0.001]。POP组双侧LUG均值在Valsalva状态下大于静息状态下[(3.04±0.86)cm vs (2.76±0.56)cm,P<0.05];而对照组双侧LUG均值在静息状态下与Valsalva状态下差异无统计学意义[(2.08±0.37)cm vs (2.24±0.54)cm,P>0.05]。POP组21例耻骨直肠肌存在双侧损伤,26例存在单侧损伤(右侧16例,左侧10例),7例无损伤。结论 利用TUI技术能够较好地观察POP患者的耻骨直肠肌的形态学特征。 |

Objective To assess the relativity between pelvic organ prolapse (POP) and levator-urethra gaps (LUG) with tomographic ultrasound imaging (TUI), and to discuss the morphological characters of levator ani muscle in women with POP. Methods POP group included 54 patients with POP, while control group included 58 patients with other benign gynecological diseases. The volume data of all the patients were collected using TUI at resting state and Valsalva state, and tomographic ultrasound slices were obtained at intervals of 2.5 mm using QLab software to measure LUG. Results LUG was larger in POP group than in control group at resting state ([2.76±0.56]cm vs [2.08±0.37]cm, P < 0.001) and at Valsalva state ([3.04±0.86]cm vs [2.24±0.54]cm, P < 0.001). There was no difference of LUG in control group between resting state and Valsalva state ([2.08±0.37]cm vs [2.24±0.54]cm, P > 0.05). LUG in POP group was larger at Valsalva state than that at resting state ([3.04±0.86]cm vs [2.76±0.56]cm, P < 0.05). There were 21 patients with bilateral pubovisceral muscle avulsion, 26 with unilateral avulsion (16 right side, 10 left side) and 7 without avulsion in POP group. Conclusion TUI can show the morphological characters of pubovisceral muscle in patients with POP. |
