| 目的 观察桥本病合并甲状腺乳头状癌的超声表现,探讨桥本病与甲状腺乳头状癌是否存在关系。方法 对1582例甲状腺疾病患者进行术前常规超声检查,回顾性分析其中桥本病合并甲状腺乳头状癌患者的超声声像图,并与病理结果对照。结果 桥本病合并甲状腺乳头状癌患者35例,超声正确诊断19例(19/35,54.29%)。本组甲状腺乳头状癌的发生率在桥本病患者中(35/80,43.75%)高于非桥本病患者(235/1502,15.65%),在出现结节的桥本病患者中(32/59,54.24%)高于无结节桥本病患者(3/20,15.00%),差异均有统计学意义(P均<0.05)。结论 桥本病、尤其是合并结节的桥本病较甲状腺其他良性病变更易并发甲状腺乳头状癌。超声是诊断桥本病合并甲状腺乳头状癌的重要手段。 |

| Objective To analyze the sonographic finding of Hashimoto's disease complicated with papillary thyroid cancer (PTC), and to explore the correlation between Hashimoto's disease and PTC. Methods Totally 1582 patients underwent ultrasonography before operation. The ultrasonic findings in patients with Hashimoto's disease complicated with PTC confirmed with pathology were retrospectively analyzed and compared with pathologic results. Results Among 35 Hashimoto's disease patients complicated with PTC, 19 were diagnosed correctly with ultrasound (19/35, 54.29%). The rate of PTC in Hashimoto's disease (35/80, 43.75%) was higher than in non-Hashimoto's disease (235/1502, 15.65%, P<0.05), while of Hashimoto's disease with nodules (32/59, 54.24%) was significantly higher than that of Hashimoto's disease without nodules (3/20, 15.00%, P<0.05). Conclusion Patients with Hashimoto's disease, especially with nodules, is easier to get complicated with PTC compared with other thyroid benign lesions. Ultrasound is helpful to the diagnosis of Hashimoto's disease coexistent PTC. |
