目的 探讨Xper CT成像在微小肝癌(MHCC)诊疗中的临床应用价值。 方法 对31例MHCC患者于TACE治疗前行肝脏增强CT、常规DSA及Xper CT成像。综合分析图像,确定肿瘤供血动脉后,用微导管超选至肿瘤供血动脉行TACE治疗。治疗后再次行CT检查观察碘油沉积情况,并随访评估TACE疗效。 结果 TACE后1个月,增强CT共发现47个微小癌灶,直径0.4~1.0 cm。TACE前增强CT、DSA、Xper CT对微小肝癌的检出率分别为63.83%(30/47)、78.72%(37/47)和89.36%(42/47),三者间差异有统计学意义(P>0.05)。TACE术中超选择插管成功率为95.74%(45/47)。TACE后6个月随访显示,31例患者中,完全缓解(CR)10例,部分缓解(PR)11例,稳定(SD)9例,进展(PD)1例;TACE治疗有效率[(CR+PR)/(CR+PR+SD+PD)×100%]为67.74%(21/31)。 结论 Xper CT成像对MHCC检出率较高,并可较为准确地反映肿瘤血供情况,对MHCC的诊疗具有较高价值。 |

Objective To investigate the clinical application value of Xper CT in diagnosing and treatment of micro hepatocellular carcinoma (MHCC). Methods Thirty-one patients with MHCC were underwent hepatic enhanced CT, conventional DSA and Xper CT befor TACE. TACE with micro-catheter was performed after confirming the feeding artery of the tumors by comprehensive analysis of medical images. Then CT scan was performed to observe lipiodol accumulation in tumors and to evaluate the therapeutic effect of TACE. Results Forty-seven MHCC (diameter of 0.4-1.0 cm) were observed in all 31 patients with enhanced CT 1 month after TACE. The detection rate of MHCC by enhanced CT scan, DSA and Xper CT before TACE was 63.83% (30/47), 78.72% (37/47) and 89.36% (42/47), respectively. Significant difference was found among the detection rates of different examination methods (P>0.05). The successful rate of super-selected catheterization was 95.74% (45/47). Six months after TACE, there were 10 patients of complete remission (CR), 11 of partial remission (PR), 9 of stable disease (SD) and 1 of progressive disease (PD) among all 31 patients, and the effective rate of TACE[(CR+PR)/(CR+PR+SD+PD)×100%] was 67.74% (21/31). Conclusion Xper CT can accurately detect MHCC and reflect the blood flow of MHCC, therefore having good value in the diagnosis and treatment of MHCC. |
