      目的 探讨320排容积CT全脑灌注成像在急性缺血性脑血管病(AICVD)中的应用价值。方法 对20例临床拟诊为AICVD患者,于发病24 h内行头颅CT平扫和全脑CT灌注(CTP)成像检查,获得脑血流量(CBF)、脑血容量(CBV)、平均通过时间(MTT)、达峰时间(TTP)等参数,同时获得动态CT血管成像(4D-CTA)图;于CTP检查后24 h内复查MR,分析其CTP、4D-CTA及MRI表现。结果 常规CT平扫示8例有脑缺血病灶,12例未见明显异常。全脑CTP 20例患者共发现与临床症状相对应的32个缺血灶,MRI发现26个病灶表现为梗死灶。4D-CTA显示责任血管不同程度狭窄或闭塞,20例患者中14例血管狭窄,2例闭塞,4例正常。结论 320排容积CT全脑灌注成像可同时获得CTP、4D-CTA图像,实现对AICVD的全面评价。
英文摘要:
      Objective To explore the application of whole-brain CT perfusion (CTP) imaging with 320-detector row CT in patients with acute ischemic cerebrovascular diseases (AICVD). Methods Twenty patients diagnosed as AICVD underwent plain CT and CTP within 24 h after the onset of symptoms. The parameter maps of cerebral blood flow (CBF), cerebral blood volume (CBV), mean transit time (MTT)and time to peak (TTP) were analyzed. Meanwhile dynamic CT angiography (4D-CTA) images were obtained. MR was performed within 24 h after CTP. Results Early infarctions were found in 8 patients, and 12 patients showed normal on conventional plain CT, while 32 ischemic lesions corresponding to clinical symptoms were found in whole-brain CTP. MRI confirmed that 26 lesions were infarct foci. And 4D-CTA showed vascular stenosis in 14 patients, occlusion in 2 patients, and normal in 4 patients. Conclusion Full evaluation of AICVD can be achieved using whole-brain CT perfusion imaging with 320-detector row CT.
