      目的 探讨采用MR电影成像方法观察空腹、餐后胃蠕动波及检测胃动力的可行性。方法 11名健康志愿者禁食6 h后取偏右侧仰卧位。采用磁共振FIESTA序列电影扫描。分别采集空腹及饮水后5、15、30 min图像,通过观测胃腔收缩幅度(PGC)和胃动力指数(GMI)来判断胃蠕动波和动力情况。结果 MR电影成像可以清晰捕捉到各时期的胃蠕动波。空腹状态下,4名在胃底、体部可见蠕动波,7名在胃窦部可见蠕动波。饮水后均可观察到蠕动波存在,饮水后胃底、体部和胃窦部PGC均高于空腹状态(P均=0.003),且胃窦部收缩程度更强。11名中,2名未测得明确的胃动力数据,余9名饮水后15 min GMI明显高于5 min(F=6.152,P=0.005),但在30 min时GMI有所下降,与5 min和15 min时比较差异无统计学意义(5 min:F=1.624,P=0.238;15 min:F=0.844,P=0.198)。结论 采用MR电影成像可以观察胃蠕动波活动及定量检测胃动力。
英文摘要:
      Objective To evaluate the feasibility of cine MRI for assessment of fasting and postprandial gastric peristaltic activity as well as antral motility. Methods Eleven healthy volunteers fasted for 6 h prior to the examination underwent cine MRI in a right decubitus position with steady-state acquisition (2D FIESTA) sequences. The proximal and distal parts of stomach were examined before and 5, 15, 30 minutes after ingestion. Gastric peristaltic activity and motility were quantified by calculating the percentage occlusion of gastric contractions (PGC) and the gastric motility index (GMI). Results The gastric peristaltic wave was observed clearly on cine MRI. Before ingestion, peristaltic waves appeared in proximal parts of stomach in 4 volunteers and distal parts of stomach in 7 volunteers, while after ingestion it was observed in all volunteers. After ingestion, PGC in proximal and distal parts of stomach were larger than that under fasting state (both P=0.003). The gastric motility index could not be acquired in 2 volunteers. For the other 9 volunteers, the gastric motility index at 15 min after ingestion was higher than that at 5 min (F=6.152, P=0.005), but decreased at 30 min compared with 5 min and 15min (5 min: F=1.624, P=0.238; 15 min: F=0.844, P=0.198). Conclusion Cine MRI is a reliable and noninvasive technique for quantitative measurement of gastric peristaltic activity and gastric motility.
