      目的 探讨二维及三维超声参数对不同性质颈部肿大淋巴结的鉴别诊断价值。 方法 收集131例颈部淋巴结肿大患者(169个颈部淋巴结),分为反应性增生组、淋巴瘤组及转移性淋巴结组,计算其短径与长径比(S/L)、淋巴结髓质面积与整体面积比值(Am/At)、淋巴结髓质体积与整体体积比值(Vm/Vt),分析三维血流分布类型,并建立判别函数。 结果 三维血流分布类型在3组间差异有统计学意义(P均<0.05);以Vm/Vt为变量X1,以三维血流分布类型为变量X2,以S/L为变量X3,以Am/At为变量X4,Y1、Y2、Y3分别代表反应性增生组、淋巴瘤组及转移性淋巴结组,建立3个Fisher线性判别函数方程:Y1=-1.532+1.389X1+1.806X2+0.995X3+1.236X4,Y2=-11.392+11.932X1+7.530X2+3.989X3+2.002X4,Y3=-15.531+12.377X1+7.850X2+7.344X3+3.422X4;回代法验证函数判断准确率分别为85.00%(51/60)、66.67%(38/57)、90.38%(47/52)。 结论 三维血流分布类型和Vm/Vt可作为判别颈部淋巴结病变性质的指标。

      Objective To investigate the value of 2D and 3D ultrasonic parameters in cervical lymph node lesions. Methods Totally 131 patients with 169 enlarged cervical lymph nodes were enrolled and divided into 3 groups, i.e. reactive node group, lymphoma group and metastatic node group. The short diameter and length ratio (S/L) of the enlarged lymph nodes, the medulla area and the total area ratio (Am/At), medulla volume and total volume ratio (Vm/Vt) and three-dimensional blood flow distribution type were analyzed, and the discriminant function formulas were established. Results Three-dimensional blood flow distribution types were statistical different among 3 groups (all P<0.05). Taking Vm/Vt for variable X1, three-dimensional blood flow distribution type for variable X2, S/L for variable X3, Am/At for variable X4, representing reactive node group, lymphoma group and metastatic node group, Y1=-1.532+1.389X1+1.806X2+0.995X3+1.236X4, Y2=-11.392+11.932X1+7.530X2+3.989X3+2.002X4, Y3=-15.531+12.377X1+7.850X2+7.344X3+3.422X4, with the accuracy rate of 85.00% (51/60), 66.67% (38/57), 90.38% (47/52), respectively. Conclusion Three-dimensional blood flow distribution type and Vm/Vt could be used as indexes in evaluation of enlarged cervical lymph nodes with different properties.
