      目的 比较3.0T MR多序列成像显示正常腕关节的价值。方法 采用GE Signa HDxT 3.0T MR对23只健康腕关节行FSE-FS-PDW、FSE-FS-T2W、T2*GRE、MERGE冠状位扫描,分别计算腕骨间软骨、骨髓、肌肉、TFC的SNR和软骨/骨髓、肌肉/骨髓、三角纤维软骨盘(TFC)/骨髓的对比噪声比(CNR),以四点评分法分析腕关节的TFC、腕骨间软骨、月三角韧带、舟月韧带的能见度。比较各序列诸结构间的SNR、CNR及能见度。结果 各序列间腕关节结构的SNR及CNR差异均有统计学意义(P均<0.001),MERGE和T2*GRE显著高于其他序列,FSE-FS-T2W均为最低;各序列两两比较,部分序列显示同一结构差异无统计学意义(P>0.05)。TFC、腕骨间软骨、月三角韧带、舟月韧带的能见度以MERGE最高,FSE-FS-PDW较高,各序列间差异均有统计学意义(P均<0.001);同一结构各序列两两比较,部分序列间差异有统计学意义(P<0.05)。结论 MERGE是显示正常腕骨间软骨、TFC以及月三角韧带、舟月韧带的优选序列,对于诊断腕关节软骨病变可能具有潜在应用价值。

      Objective To compare multi-sequence imaging of normal wrist joint on 3.0T MR. Methods MR scans were performed on 23 healthy wrists with FSE-FS-PDW, FSE-FS-T2W, T2*GRE and MERGE sequence on coronal plane using a GE Signa HDxT 3.0T MR scanner. Signal to noise ratio (SNR) of inter-carpal cartilage, bone marrow, muscle, triangular fibrocartilage (TFC) and contrast-to noise ratio (CNR) between cartilage and bone marrow, muscle and bone marrow, as well as TFC and bone marrow were calculated, and the visibility of various anatomic structures, including TFC, inter-carpal cartilage, triangular ligament, scapholunate ligament were analyzed using a four-point grading scale. SNR, CNR and visibility among multi-sequence images were compared statistically. Results SNR of cartilage, bone marrow, muscle, TFC and CNR between cartilage and bone marrow, muscle and bone marrow, TFC and bone marrow were significantly higher on MERGE and T2*GRE sequences than on the others, and on FSE-FS-T2W was the lowest. SNR and CNR of all the structures had statistical difference among different sequences (P<0.001), but for the same structure, some pairwise did not show statistical difference (P>0.05). Visibility of the TFC, intercarpal cartilage, triangular ligament and scapholunate ligament were the best on MERGE, and better on FSE-FS-PDW. There were statistical differenced among different sequences (P<0.001), and for the same structure, some pairwise had statistical difference (P<0.05). Conclusion For high-resolution imaging of the intercarpal cartilage, TFC, triangular ligament and scapholunate ligament of healthy wrist, MERGE sequence proves to be superior to other sequences, which may has potential value for the diagnosis of wrist diseases.
