      目的 应用二维斑点追踪显像(2D-STI)技术评价急性心肌梗死(AMI)患者行经皮冠状动脉内支架植入术(PCI)前后左心房功能的变化,探讨其临床应用价值。方法 应用STI技术测定30例AMI患者PCI术前、术后1周及术后1个月左心房储备功能、管道功能及辅泵功能各项指标的变化,并与30名正常人(对照组)比较。结果 与对照组相比,AMI患者PCI术前、术后1周及术后1个月LVEF、E、E/A均下降,A均升高,LAVmin、LAVmax、LAVp、LAAEF、mSRa均升高,mSRs、LAPEF、mSRe均下降,差异均有统计学意义(P均<0.05)。与PCI术前比较,AMI患者PCI术后1周左心房功能各项指标差异均无统计学意义(P>0.05);PCI术后1个月LVEF、E、E/A升高,A下降,LAVmin、LAVmax、LAVp、LAAEF、mSRa下降,mSRs、LAPEF、mSRe升高,差异均有统计学意义(P均<0.05)。与PCI术后1周比较,AMI患者PCI术后1个月LVEF、E、E/A升高,A下降,LAVmin、LAVmax、LAVp、LAAEF、mSRa下降,mSRs、LAPEF、mSRe升高(P均<0.05)。结论 STI技术可以评价AMI患者左心房功能及PCI术对左心房功能变化的影响。

      Objective To observe the application value of two-dimensional speckle tracking imaging (2D-STI) in evaluation on the left atrial function of patients with acute myocardial infarction (AMI) before and after percutaneous coronary intervention (PCI). Methods Indicators of left atrial function of 30 AMI patients were observed before and 1 week, 1 month after PCI with 2D-STI, and were compared with 30 normal subjects (control group). Results Compared with control group, before PCI, 1 week and 1 month after PCI, LVEF, E, E/A decreased, A increased, LAVmin, LAVmax, LAVp,LAAEF and mSRa increased, mSRs, LAPEF, mSRe decreased in AMI patients (all P<0.05). Compared with those before PCI, left atrial function indicators of AMI patients 1 week after PCI showed no significant difference (all P>0.05), while LVEF, E, E/A increased, A decreased, LAVmin, LAVmax, LAVp, LAAEF, mSRa decreased, mSRs, LAPEF, mSRe increased (all P<0.05) 1 month after PCI in AMI patients. Compared with 1 week after PCI, LVEF, E, E/A increased, A decreased, LAVmin, LAVmax, LAVp, LAAEF, mSRa decreased, mSRs, LAPEF, mSRe increased 1 month after PCI in AMI patients (all P<0.05). Conclusion Two-dimensional speckle tracking imaging can evaluate the changes of the left atrium function of patients with AMI after PCI and the influence of PCI.
