       目的 比较上肢两种不同体位对CT增强扫描中对比剂利用效果的影响。方法 选择142例接受CT胸部增强扫描患者,均于右臂留置静脉通道,单筒高压注射对比剂。按分层随机分组法,先根据年龄分为青年组(<40岁)、中年组(40~60岁)和老年组(>60岁);再根据采用的不用上肢体位分为第一体位组(仰卧位,双上臂伸直上举,与身体处于同一水平面)和第二体位组(仰卧位,双上臂上举,双前臂交叉置于额头上,留置静脉通道的右臂在上)。测量右锁骨下静脉胸廓入口内外各2~3 cm处CT值,取其平均值的差值作为评价对比剂利用效果的指标。比较不同年龄组和不同上肢体位之间对比剂的利用效果。结果 采用第一体位、第二体位时,不同年龄组的对比剂利用效果差异无统计学意义;而2种不同体位组之间比较,对比剂利用效果的差异有统计学意义(P<0.01)。结论 CT增强扫描时,采用第二体位可降低右腋静脉及右锁骨下静脉内对比剂在胸廓外的留滞,有助于提高对比剂的利用率,降低对比剂局部滞留引起的伪影。
英文摘要:
      Objective To compare the impact of upper limbs positions on utilization of contrast agent in contrast enhanced CT. Methods Totally 142 patients underwent contrast-enhanced thoracic CT examination. Using stratified randomization, The patients were divided into 3 groups according to age,i.e.the youth group (<40 years old), midlife group (40-60 years old) and older group (>60 years old).Then the patients were divided into 2 groups according to upper limbs positions. The first position group was supine position, the patients' double arm unbended at the same level of body, while the second position group was supine position, patients lifted double upper arms, forearms crossing on forehead, with the intravenous route side of right above the other side. The difference between mean CT values at 2-3 cm of right subclavian vein inside and outside of the entrance of thoracic cage was measured and regarded as the evaluation index of utilization of contrast agent. The utilization of contrast agent among age groups and different upper limbs positions was compared. Results There was no statistical difference in utilization of contrast agent on the two positions among different age groups. However, there was statistically significant difference in utilization of contrast agent between the two positions (P<0.01). Conclusion During contrast-enhanced thoracic CT scanning, taking the second position can decrease the accumulation of contrast agents within the right axillary vein and right subclavian vein outside the chest, which helps to improve the utilization of contrast agent and reduce local artifacts caused by accumulation of contrast agent.
