目的 评价三维超声壁龛显像在鉴别良恶性胃溃疡中的实用性和准确性。方法 回顾分析59例经病理证实的良恶性胃溃疡的三维超声壁龛显像特点,分析下列参数及指征(分别定义为X1~X9):溃疡形态、溃疡底部是否光滑、黏膜下层是否消失、有无黏膜纠集现象、胃腔有无狭窄、胃周淋巴结是否肿大、溃疡直径、溃疡周围环形软组织带宽度、周围胃壁厚度,并进行统计学分析。结果 良恶性胃溃疡在溃疡周围胃壁厚度及软组织带宽度方面差异均有统计学意义(P均<0.05),黏膜下层是否消失(X3)、黏膜纠集现象的有无(X4)、环形软组织宽度(X8)、周围胃壁厚度(X9)在鉴别良恶性溃疡中具有重要作用,Logistic回归方程为ln(P/1-P)=-21.292+20.301X3 +2.845X4+20.930X8+17.493X9。结论 三维超声壁龛显像在鉴别良恶性胃溃疡中有重要价值。 |

Objective To evaluate the practicability and accuracy of three-dimensional ultrasound in the differential diagnosis between benign and malignant gastric ulcer with niche mode. Methods In niche mode, ultrasonic manifestations of 59 patients with benign or malignant gastric ulcer confirmed pathologically were analyzed retrospectively. The contents observed included the shape (X1) and bottom (X2) of the ulcers, submucosa (X3), mucosal convergence sign (X4), gastric stenosis (X5), lymph nodes around stomach (X6), the diameter (X7), the width of soft tissue around ulcers (X9) and thickness of stomach wall (X9). All the data were analyzed statistically. Results The difference between benign and malignant gastric ulcer was significant in width of soft tissue around ulcers and thickness of stomach wall (all P<0.05). Submucosa, mucosal convergence sign, the width of soft tissue around ulcers and thickness of stomach wall played an important role in the differential diagnosis between benign and malignant gastric ulcer, and the Logistic regression equation was ln(P/1-P)=-21.292+20.301X3+2.845X4+20.930X8+17.493X9. Conclusion In niche mode, three-dimensional ultrasound is valuable in the differential diagnosis between benign and malignant gastric ulcer. |
