       目的 应用血管回声跟踪(ET)技术评价多发性骨髓瘤患者的颈动脉弹性功能。 方法 选取临床确诊的初发多发性骨髓瘤患者34例(多发性骨髓瘤组),记录其DS和ISS分期情况。另设正常对照组35名。应用ET技术检测两组双侧颈总动脉弹性系数(Ep)、硬化参数(β)、顺应性(AC)、单点脉搏波传导速度(PWVβ)和增大指数(AI)及内-中膜厚度(IMT)。 结果 多发性骨髓瘤组IMT较正常对照组增厚,但差异无统计学意义(P>0.05);与正常对照组比较,多发性骨髓瘤组β、Ep、PWVβ明显增高,AC明显降低(P<0.05)。按DS分期,多发性骨髓瘤组ⅢA期、ⅢB期AC均较ⅡA期减低,ⅢB期PWVβ较ⅡA期增加(P<0.05);按ISS分期,Ⅲ期AC、AI均较Ⅰ期减低,Ⅲ期PWVβ较Ⅱ期增加(P<0.05)。 结论 ET技术可较为敏感地反映多发性骨髓瘤患者颈动脉弹性减低。

      Objective To evaluate carotid artery elasticity function by E-tracking (ET) technique in patients with multiple myeloma (MM). Methods Ultrasonic ET method was used to measure the stiffness parameter (β), pressure-strain elasticity modulus (Ep), arterial compliance (AC), argumentation index (AI), one-point pulse wave velocity (PWVβ) and intima-media thickness (IMT) of the common carotid arteries in 34 patients with MM (MM group) and in 35 healthy subjects (control group). Results Compared with control group, IMT of carotid artery was thicker in MM group, though no statistical different was found (P>0.05). Ep, β and PWVβ were significantly higher and AC was significantly lower in MM group than those of control group (P<0.05). According to DS staging system, parameters of AC in ⅢA stage and ⅢB stage were all significantly lower than those of ⅡA stage, and PWVβ in ⅢB stage was significantly higher than that of ⅡA stage (P<0.05). According to ISS staging system, parameters of AC and AI in Ⅲ stage were significantly lower than those of Ⅰ stage, and PWVβ in Ⅲ stage was significantly higher than that ofⅡ stage (P<0.05). Conclusion The elasticity decrease of the carotid artery can be detected sensitively with ET technique.
