| 目的 探讨MSCT静脉成像(MSCTV)对髂静脉压迫综合征(IVCS)的诊断价值。 方法 对28例临床怀疑IVCS的患者先后行下肢深静脉顺行造影及直接法髂静脉MSCTV检查,评价两种方法对髂静脉的显示能力;以DSA或手术结果为"金标准",计算并比较两种检查方法诊断IVCS的敏感度及特异度。 结果 28例中,经直接法髂静脉MSCTV检查,髂静脉显示清晰26例,显示不清2例;经下肢深静脉顺行造影检查,髂静脉显示清晰19例,显示不清9例;差异有统计学意义(χ2=5.543,P=0.019)。直接法髂静脉MSCTV对IVCS的诊断敏感度及特异度分别为95.65%(22/23)、80.00%(4/5),下肢深静脉顺行造影的诊断敏感度及特异度分别为65.22%(15/23)、80.00%(4/5),敏感度差异有统计学意义(P=0.022)。 结论 直接法髂静脉MSCTV诊断IVCS具有较高敏感度,有助于检出IVCS。 |

| Objective To assess the diagnostic value of multi-slice CT venography (MSCTV) in iliac vein compression syndrome (IVCS). Methods Direct MSCTV of iliac vein was performed to totally 28 suspected IVCS patients after ascending venography of lower extremity veins. The capabilities of the two methods in displaying iliac vein were evaluated. Taking the results of DSA and surgery as the gold standard, the sensitivity and specificity of direct MSCVT of iliac vein and ascending venography of lower extremity veins were calculated and compared. Results Among all 28 cases, iliac vein was demonstrated clearly in 26 cases and demonstrated unclearly in 2 cases by direct MSCTV of iliac vein, while iliac vein was demonstrated clearly in 19 cases and demonstrated unclearly in 9 cases by anterograde venography of lower extremity veins. Statistical difference was found between the two methods (χ2=5.543, P=0.019). In diagnosis of IVCS, the sensitivity and specificity of direct MSCTV of iliac vein was 95.65% (22/23) and 80.00% (4/5), of anterograde venography of lower extremity veins was 65.22% (15/23) and 80.00% (4/5), respectively. There was statistical difference of the sensitivities(P=0.022). Conclusion Direct MSCTV of iliac vein can be used to diagnose IVCS with high sensitivity. It is helpful to improve the detection rate of IVCS. |
