目的 探讨DTI在研究颈椎病颈部肌群损伤中的应用价值,观察颈部肌群在颈椎病发病机制中的作用。方法 对30例患者(颈椎病组)与25名健康志愿者(对照组)行常规MRI和DTI,以肌腹中心作为ROI,分别测量不同肌肉相应ROI内的ADC值和FA值,并比较颈椎病患者与健康人对应肌肉ADC值和FA值的差异。结果 颈椎病组与对照组左侧颈半棘肌的ADC值差异具有统计学意义(P<0.05);而右侧颈半棘肌、两侧头半棘肌、两侧头颈夹肌、两侧胸锁乳突肌和两侧斜角肌的ADC值差异无统计学意义(P>0.05)。颈椎病组与对照组两侧颈半棘肌、两侧头半棘肌、两侧头颈夹肌、两侧胸锁乳突肌和两侧斜角肌的FA值差异无统计学意义(P>0.05)。结论 颈椎病患者部分颈部肌群有受损表现。颈部肌群弥散受限在颈椎病的发病机制中具有一定作用。 |

Objective To explore the application value of DTI in injury of neck muscle group in patients of spondylopathy, and to observe the role of neck muscle group in pathogenesis of cervical spondylopathy. Methods Thirty spondylopathy patients (cervical spondylopathy group) and 25 healthy subjects (control group) underwent conventional MRI and DTI. Cervical muscle belly center was regarded as ROI. ADC and FA values were calculated and compared between two groups. Results Significant difference was found in ADC value of left neck semispinalis (P<0.05), whereas no difference was found in ADC value of right neck semispinalis, bilateral semispinalis capitis, splenius capitis and neck, sternocleidomastoid muscle and scalenus between the two groups (P>0.05). There was no statistical difference in FA value of above muscle groups between the two groups (P>0.05). Conclusion Some part of cervical muscle group has been injured in cervical spondylosis patients. The limited diffusion of the cervical muscle group in cervical spondylopathy patients plays a role in the pathogenesis of cervical spondylopathy. |
