       目的 利用脉冲组织多普勒超声(PW-TDI)评价妊娠高血压综合征(PIH)对胎儿心脏功能的影响。方法 选择PIH孕妇62例(PIH组),正常妊娠孕妇60名(对照组),在胎儿心脏四腔切面,应用PW-TDI测量左心室(LV)侧壁、右心室(RV)侧壁、室间隔(IVS)与房室环交界处心肌运动速度,获得舒张早期E波峰值速度(Em)、舒张晚期A波峰值速度(Am)、收缩期S波峰值速度(Sm),计算Em/Am比值,进行统计分析。结果 随着孕周增大,对照组胎儿RV、LV的Em增高,IVS、LV的Sm增高,RV、IVS的Em/Am增大(P均<0.05),PIH组胎儿LV、RV、IVS的Em、Sm升高,RV、LV的Am升高,RV、IVS的Em/Am降低,LV的Em/Am升高(P均<0.05)。与相同孕周正常胎儿相比,PIH组胎儿LV、RV、IVS的Em、Am、Em/Am差异有统计学意义(P均<0.05);≥30周的胎儿中,PIH组较对照组RV的Sm增高(P<0.05),<30周的胎儿中,PIH组较对照组IVS、LV的Sm降低(P均<0.01)。结论 PIH患者胎儿心脏舒张功能与收缩功能受损。PW-TDI可评价PIH患者胎儿心脏形态和功能。

      Objective To observe the value of pulsed wave tissue Doppler imaging (PW-TDI) in the evaluation on fetal heart function in pregnancy-induced hypertension (PIH). Methods Sixty-two patients with PIH (PIH group) and 60 normal pregnant women (control group) were enrolled. From four-chamber view, ventricular myocardial velocities were measured at the basal pans of the left ventricular (LV) lateral wall, right ventricular (RV) lateral wall and inverventricular septum (IVS) with PW-TDI. Ventricular myocardial velocities included systolic myocardial velocity (Sm), early diastolic myocardial velocity (Em), late diastolic myocardial velocity (Am) and the Em/Am ratio were statistical analyzed. Results In fetuses of control group, the Em of RV, LV, the Sm of IVS, LV and Em/Am of RV, IVS increased with increasing gestational week (P<0.05). In PIH group, the Em, Sm of LV, RV, IVS and the Am of RV, LV increased, the Em/Am of RV, IVS decreased, the Em/Am of LV increased (all P<0.05). Compared with fetuses of the same gestational week in control group, the Em, Am, Em/Am of LV, RV, IVS in fetuses of PIH group were significantly different (all P<0.05). In fetuses ≥30 gestational week, the Sm of RV of PIH group was higher than that of control group (P<0.05), while in fetuses <30 gestational week, the Sm of IVS, LV of PIH group were lower than those of control group (all P<0.01). Conclusion Both systolic and diastolic function of heart are damaged in PIH fetuses. PW-TDI can evaluation the heart structure and function of PIH fetuses.
