目的 探讨DWI、ADC、指数表观扩散系数(eADC)对轻症急性胰腺炎(MAP)的诊断价值。 方法 收集临床确诊的MAP患者19例(MAP组)及正常对照者24名(NC组),均接受MR和DWI扫描(b值为0、600 s/mm2)。采用双盲法对DWI、MRI诊断MAP阳性率进行比较;分别测量MAP组、NC组的ADC、eADC值并进行分析,评估两者的95%可信区间;对MAP组ADC、eADC值与MR检查当日的血清淀粉酶、脂肪酶进行相关性分析。 结果 以竖脊肌DWI信号为参照,MAP组胰腺组织DWI呈高或明显高信号,而NC组呈等或稍高信号,DWI诊断MAP阳性率为84.21%(16/19),明显高于常规MRI(9/19,47.37%,χ2=5.14,P<0.05);MAP组、NC组ADC均值分别为(1.76±0.44)×10-3 mm2/s、(2.13±0.48)×10-3 mm2/s,eADC均值分别为0.20±0.08、0.15±0.07,差异均有统计学意义(U=0.01、0.02,P均<0.05);MAP组中ADC、eADC值与血清淀粉酶、脂肪酶无相关性(rADC=-0.06、-0.04,reADC=0.03、0.01,P均>0.05)。 结论 MAP在DWI主要表现为高信号。DWI诊断MAP优于常规MRI,并有可能作为疗效评估的客观指标。 |

Objective To observe the value of MR DWI, ADC and exponent apparent diffusion coefficient (eADC) in the diagnosis of mild acute pancreatitis (MAP). Methods Nineteen patients of MAP (MAP group) diagnosed clinically and 24 normal controls (NC group) were collected. MRI and DWI were performed, with b value of 0 and 600 s/mm2 in DWI. The positive rate of MAP diagnosed with MRI and DWI with double blind method was compared. The ADC, eADC value of both groups were measured and analyzed, and the 95% confidence interval was estimated, respectively. In MAP group, correlation of ADC and eADC with corresponding serum amylase and lipase was analyzed. Results Taking DWI signal of erector spinae as reference, DWI of pancreas tissue showed hyperintense or obvious high signal in MAP group, while isointense or slight hyperintense signal in NC group. The positive rate of MAP was 84.21% (16/19) diagnosed with DWI and 47.37% (9/19) with MRI (χ2=5.14, P<0.05). ADC value in MAP and NC group was (1.76±0.44)×10-3 mm2/s and (2.13±0.48)×10-3 mm2/s, eADC value was 0.20±0.08 and 0.15±0.07 respectively, both had statistical difference(U=0.01, 0.02, both P<0.05). The ADC, eADC value in the MAP group showed no correlation with the serum amylase and lipase (rADC=-0.06, -0.04, reADC=0.03, 0.01, all P>0.05). Conclusion MAP shows hyperintense signal on DWI. DWI is superior to routine MRI in diagnosis of MAP, and can probably be used as an objective index for evaluating the therapeutic effect of MAP. |
