| 目的 评价糖尿病肾病、非糖尿病肾病患者在透析前及不同透析时间段的肱动脉内皮功能指标,观察糖尿病肾病透析患者的血管内皮功能及随透析时间的变化规律。 方法 利用高频多普勒超声,计算透析前和不同透析时间段(透析前、透析12个月、透析36个月)以糖尿病(DN组,n=60)和其他疾病(NDN组,n=60)为原发病的尿毒症患者和正常成人(n=20)的肱动脉扩张系数(DC)、僵硬度(SD)及血流介导的内径扩张值(FMD)。 结果 ①DN、NDN组透析36个月肱动脉DC值、FMD值小于透析12个月,SD值大于透析12个月,差异有统计学意义(P<0.05);②与对照组比较,DN组、NDN组DC值和FMD值减小,SD值增大(P<0.05);透析前及各透析时间段组间比较,DN组肱动脉DC值、FMD值小于NDN组,SD值大于NDN组(P<0.05)。 结论 糖尿病肾病血液透析患者内皮功能损害程度较其他原发病患者更为严重,且随透析时间延长继续恶化。 |

| Objective To observe the endothelial function of brachial artery of patients with diabetic nephropathy (DN) or nephrosis caused by other diseases in different hemodialysis time, and to evaluate the effect of hemodialysis on endothelial function in patients with diabetic nephropathy and different hemodialysis time. Methods Distensibility coefficient (DC), stiffness degree (SD) and flow-mediated dilatation (FMD) were calculated by using high resolution ultrasound in patients with nephrosis caused by diabetes (DN group, n=60) or other diseases (NDN group, n=60) in different hemodialysis time (pre-hemodialysis, 12 months and 36 months of hemodialysis). Twenty normal adults were enrolled as control group. Results ①In DN group and NDN group, compared with patients with hemodialysis for 12 months, patients with hemodialysis for 36 months had lower DC and FMD level, while had higher SD level (P<0.05). ②Compared with those of the control group, DC, FMD of DN group and NDN group were significantly lower, while SD were higher (P<0.05). Compared with patients in NDN group during the same time of hemodialysis, DC and FMD level of DN group were lower and SD level were higher (P<0.05). Conclusion The injury of endothelial function in hemodialysis patients with DN is more serious than hemodialysis patients with other diseases, and will become worse and worse with the hemodialysis time extension. |
