       目的 通过与右心导管测值比较,评价超声心动图测量肺动脉高压血流动力学参数的临床价值。 方法 对肺动脉高压患者60例测量超声心动图指标,包括三尖瓣反流压差、肺动脉瓣舒张期最大反流压差、肺动脉瓣舒张末期反流压差、下腔静脉最大内径、下腔静脉最小内径,计算肺动脉收缩压(SPAP)、肺动脉舒张压(DPAP)、肺动脉平均压(MPAP)、右心输出量(RCO),并与右心导管的测值比较。 结果 超声心动图测量肺动脉压力与右心导管测值之间有良好相关性(P<0.001);两种方法测得的右心输出量存在相关性(P<0.001)。 结论 超声心动图测量肺动脉压力与右心导管测值有良好相关性,为临床无创测量及研究肺循环疾病血流动力学提供了可能。
英文摘要:
      Objective To observe the clinical value of echocardiography in measurement of homodynamic parameters in patients with pulmonary hypertension in comparison with right-heart catheterization (RHC). Methods Sixty patients with pulmonary hypertension were enrolled. Echocardiographic indexes included pressure gradient of tricuspid valve regurgitate, pressure gradient of peak pulmonary regurgitate, pressure gradient of end-diastolic pulmonary regurgitate, maximum diameter of inferior vena cava, minimum diameter of inferior vena cava. Systolic pulmonary artery pressure (SPAP), diastolic pulmonary artery pressure (DPAP), mean pulmonary artery pressure (MPAP) and right cardiac output (RCO) were calculated and compared with those obtained with RHC. Results The pressure of pulmonary artery measured with echocardiography significantly correlated with those obtained with RHC (P<0.001). There was correlation of RCO between the two methods (P<0.001). Conclusion There is good correlation in the pressure of pulmonary artery measured with echocardiography and RHC. Echocardiography brings the possibility of non-invasive measurement of hemodynamics in patients with lung circulation diseases.
