目的 利用定量门控99mTc-MIBI心肌显像评价左心室室壁各节段运动对左心功能的影响。 方法 对768例患者进行门控99mTc-MIBI心肌显像,采用QGSPECT程序定量获得20节段的局部室壁运动(WM)及左心室射血分数(LVEF)值。分析LVEF与心室各节段WM的关系。 结果 所得4个公共因子分别代表不同的节段信息,F1反映了心尖、侧壁的近心尖和中段、前壁、下壁和下间隔的近心尖的信息。F2反映了前壁和下间隔的中段、整个前间隔的信息。F3反映了前壁和下侧壁的基底段的信息。F4反映了下壁的中段和基底段及下间隔基底段的信息。利用多元线性回归分析的方法得出其对LVEF影响的大小顺序为F1>F3>F2>F4(标准回归系数分别为0.633、0.471、0.415、0.169,P<0.001)。 结论 应用定量门控心肌显像获得的WM与LVEF有显著的相关性,影响最大的包括心尖、侧壁的近心尖和中段、前壁、下壁和下间隔的近心尖。 |

Objective To estimate the effect of each regional wall motion on left cardiac function by using quantitative gated 99mTc-MIBI myocardial perfusion SPECT. Methods A total of 768 patients with different left ventricular function were enrolled. The regional wall motion (WM) and left ventricular ejection fraction (LVEF) were evaluated with automatic gated myocardial perfusion imaging processing software, regional WM were quantified automatically and expressed in millimeters for 20 segments based on the functional polar map. The relationship between LVEF and each segment was analyzed. Results The acquired 4 common factors represented information of different segments respectively. The factor 1 (F1) reflected the information of apex, distal of lateral wall and middle, distal of anterior, inferior and inferior-septum wall. The factor 2 (F2) reflected the information of middle of anterior and inferior-septum, anterior-septum wall. The factor 3 (F3) reflected the information of distal of anterior and lateral wall. The factor 4 (F4) reflected the information of middle inferior, distal inferior and distal inferior-septum wall. Then the correlation between LVEF and 4 factors was analyzed by using multivariable linear regression methods. The influence of each segment on left ventricular function was different according to the standardized partial regression coefficient, which was F1>F3>F2>F4 (standardized coefficient β was 0.633, 0.471, 0.415, 0.169, respectively; P<0.001). Conclusion The correlation between LVEF and WM is good, especially in apex, middle and distal of lateral wall, distal of anterior wall, inferior and inferior-septum wall. |
