目的 通过DTI观察早期帕金森病(PD)患者小脑微观解剖结构的变化情况,探讨相应功能改变及发病机制。方法 PD组为25例符合PD诊断标准的患者,包括震颤亚组12例,非震颤亚组13例;选取20名健康志愿者作为对照组。对两组受检者进行常规颅脑MR检查和DTI检查,分别计算出全脑的FA图及平均弥散率(MD)图。采用基于体素依赖分析(VBA)方法的SPM 5软件分别比较PD组和对照组、震颤亚组和非震颤亚组的FA图和MD图。结果 与对照组相比,PD组右侧小脑后叶FA值增高,MD值减低;与非震颤亚组相比,震颤亚组FA值升高的脑区包括左侧小脑前叶、右侧小脑后叶及脑桥,MD值减低的脑区包括左侧小脑前后叶、右侧小脑后叶及脑桥。结论 PD患者小脑结构的完整性在发病早期已发生改变,小脑可能对PD患者的功能障碍起到代偿作用,并参与静止性震颤的调节。 |

Objective To investigate the microstructure abnormality of cerebellum in patients with early-stage Parkinson disease (PD) with DTI, and to explore the corresponding functional changes and its pathogenesis. Methods Twenty-five PD patients with (resting tremor subgroup, n=12) and without (without resting tremor subgroup, n=13) resting tremor were enrolled as PD group. Control group included 20 healthy volunteers. All the subjects underwent routine MRI and DTI. FA and mean diffusivity (MD) maps were obtained after post-processing and compared with voxel-based analysis (VBA) in SPM 5 between PD group and control group, with and without resting tremor subgroups statistically. Results Compared with control group, patients in PD group showed increased FA and decreased MD in posterior lobe of right cerebellum. Compared with the without resting tremor subgroup, patients in resting tremor subgroup showed increased FA and decreased MD in pons and bilateral cerebellum. Conclusion Early-stage morphological changes in the cerebellum of PD patients. The cerebellum could play a compensatory role during PD dysfunction, and involves the regulation of rest tremor. |
