| 目的 探讨原发性肺肉瘤样癌的CT表现特征。 方法 回顾性分析经手术病理证实的13例原发性肺肉瘤样癌的CT表现特点。所有病例术前均接受胸部CT平扫,其中10例接受增强扫描。 结果 13例均表现为肺内单发肿块;右肺7例,左肺6例;中心型4例,周围型9例;2例肿瘤形态规则,边界尚清,11例形态欠规则,边界不光整;病灶最大径3.0~8.3 cm,平均4.9 cm;肿块密度不均匀,10例增强后肿块均呈周边厚薄不均的环形强化,中央强化不明显或不均匀斑片状强化。3例侵及胸膜或胸壁组织,5例可见肺门和(或)纵隔淋巴结肿大,1例伴有单侧胸腔少量积液。 结论 原发性肺肉瘤样癌的CT表现具有一定的影像学特征,确诊仍需依靠组织病理学检查。 |

| Objective To investigate the CT features of primary pulmonary sarcomatoid carcinoma (PSC). Methods CT manifestations of 13 patients with primary PSC confirmed by pathology were reviewed retrospectively. All patients underwent plain CT examinations, while 10 of them also underwent contrast-enhanced CT scan before surgery. Results All lesions were unilateral. Seven lesions located in right lung and 6 lesions occurred in left lung, including 9 were peripheral and 4 central tumors. The lesions appeared regular in 2 cases with well-defined margins, but irregular in 11 cases with ill-defined margins. The mean maximum diameter of the lesions was 4.9 cm (range, 3.0 to 8.3 cm). The tumor showed heterogeneous density in all cases. Ten showed irregular peripheral heterogeneous contrast enhancement after injection of contrast media. The center part of the tumor showed no enhancement or inhomogeneous enhancement. Three lesions had thoracic wall or pleural invasion, 5 had hilar or mediastinal lymphopathy and 1 had unilateral pleural effusion. Conclusion CT features of primary pulmonary sarcomatoid carcinoma are distinctive in certain extent, but the final diagnosis still depends on histopathological examination. |
