       目的 探讨超声二维斑点追踪显像(2D-STI)技术评价冠心病患者左心房功能的临床应用价值。方法 应用Simpson's法测量冠心病组(n=28)左心房最大容积(LAVmax)、左心房最小容积(LAVmin)、左心房P容积(LAVP),计算左心房主动射血分数(LAAEF)和左心房被动射血分数(LAPEF)。应用2D-STI技术获取距二尖瓣环6~8 mm的左心房侧壁和房间隔处的应变率曲线,测定各房壁的左心室收缩期峰值应变率(SRs)、舒张早期峰值应变率(SRe)、舒张晚期峰值应变率(SRa),计算其平均值(mSRs、mSRe、mSRa),并与对照组(n=30)比较。结果 与对照组比较,冠心病组LAVmax、LAVmin 、LAVP、LAAEF显著增高(P均<0.05),LAPEF显著减低(P<0.05);mSRs、mSRe均减低(P均<0.05),mSRa增高(P<0.05)。检查者间变异系数为6.22%~7.91%,检查者内变异系数为5.31%~7.42%。结论 应用2D-STI技术评价冠心病患者左心房功能可行且重复性好。

      Objective To observe the clinical value of evaluation on the function of the left atrial with two-dimensional speckle tracking imaging (2D-STI) in patients with coronary artery diseases (CAD). Methods For CAD group (n=28) and control group (n=30), the maximal and minimum left atrial volume (LAVmax and LAVmin) were measured, and left atrial passive ejection fraction (LAPEF) and left atrial active ejection fraction (LAAEF) were calculated using Simpson's method. The sampling volume was placed on the left lateral atrial wall and interatrial septum 6—8 mm away from the annulus of mitral valve to obtain the strain rate curve. Peak strain rate indices (SRs, SRe and SRa) were measured with 2D-STI, and the mean peak strain rates (mSRs, mSRe and mSRa) were calculated. Results Compared with those of the control group, LAVmax, LAVmin, LAVP and LAAEF of CAD group significantly increased (all P<0.05), while LAPEF of CAD group significantly decreased (P<0.05). The mSRs and mSRe of left ventricular in CAD group were lower than those in control group (both P<0.05), while mSRa increased compared with that in control group (P<0.05). Coefficients of variation were 6.22%—7.91% (inter-examiner) and 5.31%—7.42% (intra-examiner). Conclusion 2D-STI can be used to evaluate the left atrium function of patients with CAD.
