目的 观察兔肝急性缺血再灌注损伤(I/R)CT灌注参数的演变规律及与肝脏酶学指标(AST、ALT)的相关性。方法 新西兰大白兔24只,随机分成4组(0.5 h、2.0 h、6.0 h缺血再灌注组与对照组,每组6只),I/R组结扎肝左叶血供60 min后恢复,制备I/R模型。各组分别接受全肝CT灌注成像,在灌注图上测量肝动脉灌注量(HAP)、门静脉灌注量(HPP)、总灌注量(TLP)及肝动脉灌注指数(HPI)。检查结束后,经下腔静脉取血样,测定AST、ALT。结果 与对照组比较,HAP在0.5 h组显著降低(P<0.05),而在2.0 h组急剧升高(P<0.01);HPP在各I/R组均低于对照组(P均<0.01);TLP在0.5 h、6.0 h 组与对照组比较差异有统计学意义(P均<0.01); I/R各组HPI与对照组比较显著升高(P均<0.01);各I/R组血清AST、ALT显著高于对照组(P均<0.05)。AST与HPI之间存在显著相关性(r=0.565, P<0.05);ALT与HAP、HPI、HPP均有相关性(r=0.418,P<0.05;r=0.767,P<0.01;r=-0.674,P<0.01)。结论 通过CT灌注参数,能够动态监测兔肝I/R后血流动力学改变。 |

Objective To observe the changes of hepatic CT perfusion parameters and the correlation with serum amylase(ALT, AST) in rabbit hepatic ischemia reperfusion injury (I/R) models. Methods New Zealand rabbits underwent 60 min of left lobar ischemia followed by 0.5, 2.0, and 6.0 h of reperfusion (each n=6). Six rabbits were taken as normal controls. All the rabbits underwent CT perfusion protocol. The perfusion indices of hepatic arterial perfusion (HAP), hepatic portal perfusion (HPP), total liver perfusion (TLP) and hepatic perfusion index (HPI) were measured after obtaining perfusion index maps on the workstation. Blood samples were taken from the superior mesenteric vein to measure the levels of aspartate and alanine transaminase (ALT and AST) at different time points. Results HAP was significantly lower in 0.5 h group than that in the control group (P<0.05), but significantly increased in 2.0 h group (P<0.01). HPP of I/R groups were significantly lower than control group (all P<0.01). TLP changed significantly in the 0.5 h and 6.0 h groups compared to that in control group (both P<0.01). HPI values were higher in I/R groups than in control group (all P<0.01). In addition, AST and ALT in I/R groups were higher than those in the control group (all P<0.05). The difference of HPI was closely related to the increase of AST (r=0.565, P<0.05), while HAP, HPI and HPP of I/R groups correlated with ALT, with correlation coefficients of 0.418 (P<0.05), 0.767 (P<0.01) and -0.674 (P<0.01), respectively. Conclusion CT perfusion is sensitive to detect the hemodynamic changes with perfusion parameters in acute rabbit hepatic ischemia reperfusion injury models. |
