目的 探讨经动脉导管超声印压检测梗死心肌硬度的可行性,并观察其与心肌胶原含量和Ⅰ/Ⅲ型胶原比值的关系。方法 将18只犬随机分为心肌梗死组(n=9)及假手术组(n=9),通过股动脉或颈动脉途径插管,将7F超声印压导管送入左心室,印压测量左心室前壁和心尖部硬度,并测量相应区域心肌胶原容积分数(CVF)、积分光密度(IOD)和Ⅰ、Ⅲ型胶原CVF比值(CVFⅠ/CVFⅢ)。结果 心肌梗死组前壁、心尖部心肌硬度值分别为(140.69±30.71)kPa、(49.08±6.17)kPa,均显著高于假手术组 。两组犬前壁心肌应力和相关形变均呈线性关系(心肌梗死组Y=138.17X,r=0.90,假手术组Y=18.69X,r=0.84,P均<0.05)。梗死组前壁、心尖部心肌CVF、IOD、CVFⅠ/CVFⅢ与假手术组相比均显著升高(P均<0.05)。结论 介入超声印压能够区分活体犬梗死心肌与正常心肌之间的硬度差异。 |

Objective To investigate the feasibility on trans-arterial stiffness assessment of myocardial infarction in canine with ultrasound indentation catheter, and to observe it's correlation with collagen content and Ⅰ/Ⅲ collagen ratio. Methods Eighteen canines were randomly divided into myocardial infarction group and sham group (each n=9). Ultrasound indentation catheter (7F) was delivered into left ventricle via the femoral artery or the carotid artery to detect the stiffness on the left ventricular anterior wall and apex, meanwhile the collagen volume fraction (CVF), integrity optical density (IOD) of collagen and CVF ratio of Ⅰ/Ⅲ collagen were also measured. Results Myocardial stiffness on the anterior wall and apex was (140.69±30.71)kPa, (49.08±6.17)kPa, respectively in myocardial infarction group, which were higher than those in sham group . There was linear relationship between stress and deformation at anterior wall in myocardial infarction group (Y=138.17X, r=0.90, P<0.05) and sham group (Y=18.69X, r=0.84, P<0.05). The CVF, IOD, CVFⅠ/CVFⅢ at anterior wall and apex in myocardial infarction group were obviously higher than those in sham group (P<0.05). Conclusion Trans-arterial ultrasound indentation catheter can be used to distinguish the stiffness variance between myocardial infarction and normal myocardial of canine in vivo. |
