       目的 探讨不同年龄段急性期川崎病(KD)合并冠状动脉非扩张性损害患儿左前降支(LAD)血流动力学改变的彩色多普勒超声特征。方法 分析31例急性期川崎病合并冠状动脉非扩张性损害患儿(KD组)的LAD血流彩色多普勒超声表现,分析<1岁、1~3岁和>3岁3个亚组KD患儿急性期LAD近段内径、血流量、舒张期峰值血流速度(PDV)、平均血流速度(MDV)、速度时间积分(VTI),并分别与27名正常儿童(正常对照组)比较。结果 不同年龄段急性期非冠状动脉扩张性KD患儿,其LAD近段血流参数PDV、MDV及VTI均高于正常对照组(P均<0.05),但LAD近段内径、血流量差异无统计学意义(P>0.05)。KD组内比较:<1岁、1~3岁及>3岁亚组LAD近段内径、血流量均随着年龄增加而增加,组内差异有统计学意义(P<0.05),而PDV、MDV及VTI差异均无统计学意义。正常对照组内比较:随着年龄的增加,LAD内径、LAD血流量增加,PDV、MDV降低,差异有统计学意义(P<0.05),但VTI差异无统计学意义。结论 KD急性期冠状动脉炎性病变可导致LAD血流速度增加;研究LAD的血流动力学有助于早期诊断和评估非冠状动脉扩张性KD的冠状动脉损害。

      Objective To investigate the hemodynamics of the left anterior descending artery (LAD) of different aged children in acute phase of Kawasaki disease (KD) with non-dilated coronary arterial lesions (nCAL) with Doppler echocardiography. Methods Totally 31 children of acute phase of KD (KD group) were enrolled and assigned into 3 subgroups according to age: <1 year, 1—3 year and >3 year. Twenty-seven normal children with matched ages were selected as normal control group. The echocardiographic manifestations of LAD were retrospectively reviewed, parameters including the diameter, flow volume, peak diastolic flow velocity (PDV), mean diastolic flow velocity (MDV), diastolic velocity time integral (VTI) were measured at the LAD artery's proximal portions. The results were analyzed and compared between two groups and among 3 subgroups of KD group. Results The PDV, MDV and the VTI significantly increased respectively in KD group of nCAL patients in acute phase in comparison with those of control group (P<0.05), but no difference of diameter nor flow volume was detected. There were statistical significances in diameter, flow volume, but not in PDV, MDV nor VTI among KD patients. In control group, both diameter and flow volume in LAD showed significant increase, while PDV and MDV showed significant decrease with increasing age (P<0.05), but VTI showed no significant difference. Conclusion KD can cause the increasing of coronary artery's velocity, and Doppler echocardiography might provide earlier diagnostic method of coronary arterial lesions in KD.
