| 目的 通过测量鼻咽癌颈部转移性淋巴结放疗前后ADC值,观察扩散加权成像对放疗疗效的评价能力。方法 选取24例经组织病理学证实的鼻咽癌伴颈部转移性淋巴结肿大患者,分别于放疗前、放疗中第7、14天及放疗结束后第4周行常规MR及扩散成像检查。根据治疗前后淋巴结的体积变化,将其分为有效组及无效组。测量各时间点ADC值。比较组内各时间点及组间相同时间点ADC值的变化。结果 放疗第7及14天,有效组淋巴结ADC值高于无效组,差异有统计学意义(P均<0.05)。放疗第7及14天,有效组淋巴结ADC值较放疗前升高,放疗第14天淋巴结ADC值较第7天亦升高,差异均有统计学意义(P均<0.05)。结论 扩散加权成像可用于评价鼻咽癌颈部淋巴结转移放射治疗的疗效。 |

| Objective To observe the efficacy of DWI for evaluating the radiotherapeutic effect for metastatic cervical lymph nodes in nasopharyngeal carcinoma patients before and after radiotherapy. Methods Totally 24 patients with nasopharyngeal carcinoma and metastatic cervical lymphadenectasis confirmed pathologically were selected. The patients underwent routine MRI and DWI before radiotherapy and on 7th and 14th day during the course, as well as 4 weeks after radiotherapy, respectively. All patients were divided into effective group and ineffective group according to volume changes of lymph nodes. The ADC values of cervical metastastic lymph nodes at each time point were measured, and were compared among time points in the same group and between same time point of different groups. Results ADC values of cervical metastastic lymph nodes in the effective group on the 7th and 14th day were higher than that in the ineffective group (all P＜0.05), also higher than that before radiotherapy, while on the 14th day were higher than that on the 7th day (all P＜0.05). Conclusion DWI can be used to evaluate efficacy of radiotherapy for metastatic lymph nodes of NPC. |
