      目的 回顾性评价三阴性乳腺癌即雌激素受体(ER)、孕激素受体(PR)和人表皮生长因子受体2(Her-2,又称CerbB-2)均为阴性的乳腺癌的MR特征性表现,并与ER阳性/PR阳性/Her-2阴性的乳腺癌MR表现做对比性研究。方法 经手术后病理证实为三阴性乳腺癌的患者18例和ER阳性/PR阳性/Her-2阴性乳腺癌患者46例纳入本研究。MR表现包括肿瘤形态、肿瘤边缘、内部强化方式和时间-信号曲线(TIC)类型。并将MR表现与病理学表现做对照分析。结果 环形强化(P<0.001)、持续上升型强化曲线(P=0.004)与三阴性乳腺癌有显著相关性。三阴性乳腺癌组肿瘤组织学级别更高(50%,9/18 vs 4.3%,2/46)。三阴性乳腺癌组与ER阳性/PR阳性/Her-2阴性组间肿瘤病理学类型间,髓样癌与三阴性乳腺癌有显著相关性(P=0.022)。结论 MRI某些特征表现对检出三阴性乳腺癌有一定的作用。
英文摘要:
      Objective To retrospectively evaluate the MRI findings of "triple negative" breast cancer (cancer that is estrogen receptor negative, progesterone receptor negative, and human epidermal growth factor receptor 2 negative) and to compare them with those of breast cancers that are ER positive, PR positive, and Her-2 negative. Methods The MR findings in 64 women with surgically confirmed triple-neagtive breast cancer (n=18) and ER positive/PR positive/Her-2 negative breast cancer (n=46) were reviewed between Feburary 2007 and December 2009. MR imaging findings included tumor shape, margin, internal enhancement, and time intensity curve (TIC). The MR imaging findings were compared with the pathologic findings. Results Rim enhancement (P<0.001), persistent enhancement pattern (P=0.004) were significantly associated with triple-negative breast cancer. Women with triple-negative breast cancer were more likely to have histologically high grade tumor (50%, 9/18 vs 4.3%, 2/46). Among the pathologic subtypes, medullary carcinoma (P=0.022) was significantly associated with triple-negative breast cancer. Conclusion Several MR imaging features might be used for detecting triple-negative breast cancer.
