       目的 探讨超声对甲状腺乳头状癌的诊断价值。方法 回顾性分析302例甲状腺乳头状癌患者的超声声像图表现,对肿块在甲状腺内的分布、大小、回声及其有否囊性变、形态和边界、钙化及其大小、血流状况进行分析,计算超声诊断符合率。结果 302例甲状腺乳头状癌中共有337个病灶,右侧病灶181个,左侧病灶150个,峡部病灶6个。超声诊断病灶最大者54.0 mm×52.0 mm×33.0 mm,最小者2.1 mm×1.6 mm×1.8 mm。甲状腺乳头状癌声像图均表现为低回声,肿块不伴囊性变比例为98.22%(331/337)。肿块存在钙化灶占61.13%(206/337),其中沙砾样钙化占51.04%(172/337)。肿块高度可大于宽度,边界清晰或不清晰,边缘和形态一般不规则或呈微分叶状。超声诊断符合率为86.09%(260/302)。结论 超声声像图对甲状腺乳头状癌有很高的诊断价值。

      Objective To evaluate the diagnostic value of ultrasonography for papillary thyroid carcinoma. Methods The ultrasonography features of 302 patients with papillary thyroid carcinoma were retrospectively reviewed, including location, size, echogenicity, shape and margin, calcification, cystic change and blood flow. The diagnostic accuracy of ultrasonography was calculated. Results Totally 337 malignant nodules were confirmed after operation in 302 patients with papillary thyroid carcinoma, with 181 nodules in right side, 150 nodeles in left side and 6 nodules in isthmus. Ultrasonography showed that the largest nodule was 54.0 mm×52.0 mm×33.0 mm and the smallest one was 2.1 mm×1.6 mm×1.8 mm. The hypoechogenicity was seen in all the nodules and 98.22% (331/337) were without cystic change. Calcifications were seen in 206 nodules (206/337, 61.13%) and gravel-like microcalcifications were found in 172 nodules (172/337, 51.04%). The nodules had a height-to-width ratio >1.0 and a microlobulated or irregular margin were also seen in papillary thyroid carcinoma. The diagnostic accuracy of ultrasonography for papillary thyroid carcinoma was 86.09% (260/302). Conclusion Ultrasonography is a feasible tool for the diagnosis of papillary thyroid carcinoma.
