| 目的 分析鼻咽癌(NPC)侵犯肌肉的MRI表现,探讨MRI在诊断NPC侵犯肌肉中的作用。方法 收集经病理证实为NPC合并头颈部肌肉受侵的72例患者的MRI资料,分析其MRI表现及与临床分期的关系。结果 腭帆张肌和腭帆提肌受侵最为多见,共67例,常见多肌肉受累,单发者少见。受侵肌肉肿胀增粗34例,变形移位23例,变性萎缩15例;其中38例呈长T1长T2信号,34例信号不均匀。32例增强扫描病灶明显强化,25例不均匀强化,环状强化6例,无强化9例。其他改变包括颅底骨质破坏40例,侵犯海绵窦21例,颈静脉孔破坏13例,舌下神经管破坏6例。结论 MRI对显示NPC伴头颈部肌肉受侵的部位、范围、浸润深度和信号改变及其准确分期具有重要价值。 |

| Objective To analyze the MRI features of muscle involvement in patients with nasopharyngeal carcinoma (NPC), and to assess the value of MRI in diagnosing muscle involvement in NPC. Methods Seventy-two patients with NPC and muscle involvement of head and neck confirmed pathologically were enrolled. MRI features and the correlation with clinical stage were analyzed. Results The tensor veli palatini and musculi levator veli palatini were involved in 67 patients, most of which were multi muscles involved. Muscular swelling was found in 34 patients, displacement and deformation in 23 patients, while muscular degeneration and atrophy were detected in 15 patients. Thirty-eight patients had low signal on T1WI and high signal on T2WI, 34 had heterogeneity of signal. Obvious enhancement was noticed in 32 patients, while uneven enhancement in 25, circular enhancement in 6 and no enhancement in 9. Other MRI features of NPC involvement included cranial base destructed (n=40), cavenous sinus invaded (n=21), jugular foramen destruction (n=13) and canalis hypoglossi destruction (n=6). Conclusion MR has great value in the diagnosis of the position, extension, invasion and characteristics of muscle involvement in patients with NPC. |
