目的 通过MSCT灌注扫描时间-密度曲线,探讨鼻咽癌放疗后局部复发与纤维化的鉴别要点。方法 53例鼻咽癌放疗后患者按照病理结果分为鼻咽癌放疗后局部复发组和纤维化组,行MSCT灌注扫描,将获得的340幅连续图像传入GE AW 4.2工作站,利用Perfusion 3软件行后处理,软件自动生成时间-密度曲线,在曲线上计算峰值及达峰时间,并进行统计学分析。结果 鼻咽部放疗后局部复发组与纤维化组的峰值、达峰时间差异有统计学意义;鼻咽癌放疗后局部复发组和纤维化组拟合后的时间-密度曲线构成比差异有统计学意义,曲线类型分为 "速升速降"型(Ⅰ型)、"速升缓降"型(Ⅱ型)、"缓升缓降"型(Ⅲ型)和"平坦"型(Ⅳ型)。联合Ⅰ和Ⅱ型拟合后的时间-密度曲线对鼻咽癌复发诊断敏感度和特异度分别为89.29%、88.00%,阳性预测值、阴性预测值分别为89.29%、88.00%。结论 鼻咽癌放疗后复发与纤维化的MSCT灌注扫描的时间-密度曲线的峰值、达峰时间以及类型不同,有助于鉴别诊断。 |

Objective To investigate the differentiation of local recurrence and fibrosis with time-density curves (TDC) of MSCT perfusion imaging in patients with post radiotherapy nasopharyngeal carcinoma. Methods Fifty-three patients with post-radiotherapy nasopharyngeal carcinoma undergoing CT perfusion examinations were divided into local recurrences group (n=28) and fibrosis group (n=25) according to pathologic findings. A total of 340 images were acquired and sent to GE AW 4.2 workstation for post-processing with Perfusion 3 software, which automatically generated fitting time-density curves (FTDC). The peak enhancement (PE) and peak time (PT) were attained on TDC. Results Significant differences of PE and PT were found between the two groups (P<0.05). There was statistical difference in constituent ratio of FTDC types (P<0.01). Combined Ⅰ type and Ⅱ type, the sensitivity, specificity, positive predictive value (PV) and negative PV of detecting local recurrences was 89.29%, 88.00%, 89.29% and 88.00%, respectively. Conclusion Patients with post radiotherapy nasopharyngeal carcinoma have difference PE, PT, and type of TDC of MSCT perfusion imaging, which will be helpful in differentiation of recurrent tumor from fibrosis. |
