目的 探讨喉全切术后患者长期使用发音管,发音重建后语言功能网络是否发生重塑。方法 采用视觉呈现事件相关设计fMRI。使16例喉全切患者(患者组)、17名正常人(对照组)完成朗读汉语中性名词任务。使用AFNI软件统计各语言功能脑区激活体素,采用SPSS 13.0统计软件对两组受试者语言功能脑区激活体素进行独立样本t检验;患者组对称激活脑区激活体素进行配对比较秩和检验。结果 患者组双侧额中回、右侧角回、右缘上回激活体素多于对照组(P<0.05)。额叶、顶叶及中央前回深部白质(10例)观察到位置固定的激活点,右顶叶皮层下(6例)激活点多发;部分患者基底节与额下回、中央前回之间及额叶观察到断续线样激活,对照组未见此表现。结论 喉全切术发音重建后右顶叶发生重塑,语言网络中某些脑区、连接点及传导通路功能强化。 |

Objective To assess the existence of plasticity of language networks of patients with stable long-term rebuilt pronunciation after total laryngectomy. Methods Event-related visually presented stimuli was employed. Sixteen patients with stable rebuilt pronunciation (patients group) and 17 healthy subjects (control group) were enrolled. The task was to read neutral Chinese bi-words loudly which presented visually. Activated voxels were obtained with AFNI software package. Statistical analysis was performed using independent samples t test for the comparison of voxel counts of related linguistic cortex between patients group and control group. Paired rank sum test was used for the comparison of voxel counts between those areas activated bilaterally of the patients. Results Statistical difference of activated voxels was found in bilateral middle frontal gyrus, right angular gyrus and right supramarginal gyrus (P<0.05) between and control group patientsgroup group and control group. Some unexpected findings were noticed in the patient group, including more than 1 activation in the white matter of the right parietal lobe of 6 patients, some dot activation in the deep white matter of the bilateral parietal cortex, frontal lobe and precentral gyrus of 10 patients, some linear continuous activation between the basal ganglia and the inferior frontal gyrus, precentral gyrus and in the frontal lobe. Conlusion Function of some areas, also some linguistic pathway and connections have been enhanced and changes of plasticity happen in the right parietal cortex of patients with long-term rebuilt pronunciation after laryngectomy. |
