      目的 探讨应用T2*map成像技术诊断腰椎间盘早期退变的可行性。 方法 青年志愿者40名(均无腰痛和腰椎部畸形、外伤、手术史),接受L1~S1椎间盘常规T2WI和T2*map扫描,生成T2*伪彩图(T2*map)。在L1/2~L5/S1的各个椎间盘髓核内放置5个感兴趣区(ROI),测量T2*值。分析并比较腰椎间盘内与盘间的髓核T2*值的差异。 结果 髓核内T2*值在同一椎间盘内的不同区域或在不同椎间盘平面内的对应区域差异有统计学意义(P<0.05);L1/2和L2/3椎间盘后点ROI的T2*值明显增高,L4/5、L5/S1位于髓核中央的ROI的T2*值升高最明显(P<0.05);T2*值在不同椎间盘平面内的对应区域,随解剖位置的降低而规律性递增(P<0.05)。 结论 髓核T2*弛豫时间值的空间变化可能量化反映椎间盘退变早期髓核的生化状态。T2*map成像技术可以为诊断腰椎间盘早期退变提供影像学依据。
英文摘要:
      Objective To approach the feasibility of MR T2*map in early detecting lumbar intervertebral disc degeneration in young volunteers. Methods Forty young volunteers without history of lumbago, lumbar operation or injury were selected. MR T2W and T2*map examination of the lumbar spine disc were performed in the L1—S1 region. T2* values at 5 regions of interest (ROI) placed in the nucleus pulposus of each lumbar intervertebral disc from L1/2 to L5/S1 were measured and statistically analyzed. Results There were statistical differences among the T2* values at the various ROI in the same lumbar intervertebral disc or at the corresponding ROI in the different anatomic site (P<0.05). T2* values that at the posterior surface ROI in L1/2 and L2/3 increased obviously (P<0.05). The changes of T2* values at the middle of nucleus pulposus in L4/5 and L5/S1 increased obviously (P<0.05). T2* values at the corresponding ROIs in different intervertebral disc level increased regularly as the anatomic site stepping down (P<0.05). Conclusion The transmutation of biochemistry in the prophase degenerative intervertebral disc might be imaged with T2* value of the nucleus pulposus. T2*map can provide imaging information of prophase degeneration of intervertebral discs.
