| 目的 探讨磁共振扩散加权成像(DWI)表观扩散系数(ADC值)对子宫内膜癌宫颈浸润的诊断价值。方法 实验组:选取经病理证实有宫颈浸润的子宫内膜癌患者26例;对照组:随机选取无宫颈浸润的子宫内膜癌患者26例。分别测量每例患者宫腔内癌灶、宫颈内口、外口的ADC值,进行ROC分析。结果 实验组宫腔内癌灶的ADC均值 ,与宫颈内口的ADC均值 差异无统计学意义(t=－0.45,P=0.66)。对照组宫腔内癌灶ADC均值 显著低于宫颈内口的ADC值 ,差异有统计学意义(t=－6.86,P＜0.001),与实验组宫腔内癌灶ADC均值比较差异无统计学意义(t=0.74,P=0.47)。以宫颈内口ADC值进行ROC分析,曲线下面积为0.86,最佳诊断界值为1.02×10－3 mm2/s,敏感度为73.1%,特异度为96.2%。结论 DWI有助于诊断子宫内膜癌宫颈浸润。 |

| Objective To observe the value of apparent diffusion coefficient (ADC) in detecting cervical involvement of endometrial carcinoma (EC). Methods Twenty-six patients with cervical involvement (CI) of EC were selected as experiment group, while 26 patients without CI were enrolled as the control group. ADC values were measured in the regions of interest within the intrauterine endometrial cancer, the internal and the external os of the cervix of all patients. Receiver operating characteristic (ROC) curve analysis was performed to investigate whether ADC values could help to discriminate CI. Results There was no significant difference of mean ADC value between intrauterine cancer ( ×10－3 mm2/s) and the cervical internal os ( ×10－3 mm2/s) in the experiment group (t=－0.45, P=0.66). The mean ADC value of the intrauterine endometrial carcinoma ( ×10－3 mm2/s) was significantly lower than that of the cervical internal os ( ×10－3 mm2/s) in the control group (t=－6.86, P＜0.001). Meanwhile, the mean ADC values of the intrauterine endometrial carcinoma showed no significant difference between the two groups (t=0.74, P=0.47). Area under curve (AUC) of ADC was 0.86. The optimal ADC threshold value was 1.02×10－3 mm2/s, the sensitivity and the specificity was 73.1% and 96.2%, respectively. Conclusion ADC measurement can provide useful information in detecting the cervical involvement of endometrial carcinoma. |
