目的 验证评估以超声造影剂声诺维(SonoVue)作为指示剂的超声造影稀释直接法作为一种测定血管通路再循环的新方法的可行性及准确性。方法 建立通路再循环体外模型,运用超声造影定量分析技术,计算再循环率测定值;根据两泵的实际设定流量,计算再循环率理论值。分析测定值与理论值的相关性及一致性,评价高、低频探头测定值间的一致性。结果 该方法敏感度和特异度均为100%;测定值与理论值有密切的相关性(r2=0.9645,P<0.0001),进行Bland-Altman检验,仅一个点在95%一致性界限以外,具有良好的一致性;高、低频探头测定值之间也具有很好的一致性,但均存在对再循环实际值的高估。结论 以SonoVue作为示踪剂的超声造影稀释直接法是一种准确、有效、敏感的透析血管通路再循环测定方法。 |

Objective To evaluate the feasibility and accuracy of a new method—ultrasonic contrast dilution-direct method for determining the accession of recirculation which used SonoVue as an indicator. Methods The measured ratios were calculated based on the quantitative analysis of video-intensity curves, and the theoretical ratios with setting flows of two pumps in access recirculation model were calculated as well. Then the correlation and consistency between theoretical and measured ratios was analyzed, and the consistency between high and low frequency probe was evaluated. Results The sensitivity and specificity was both 100%. The measured ratios correlated well with theoretical ratios (r2=0.9645, P<0.0001). In Bland-Altman test, only one spot was out of 95% confidence interval,indicating that the theoretical results are agreeable with the experimental results. The value measured with high frequency probe was in accordance with that using low frequency probe. But this method overestimated the actual recirculation value. Conclusion Ultrasonic contrast dilution-direct method is an accurate, effective and sensitive method to determine haemodialysis vascular access recirculation. |
