       目的 探讨三维能量多普勒超声对绝经后阴道流血患者子宫内膜疾病的诊断及鉴别诊断价值。方法 对绝经后阴道流血患者77例,包括子宫内膜癌35例(恶性组)和子宫内膜良性病变42例(良性组)行三维超声检查,测量病变处血流的收缩期峰值速度(PSV)、搏动指数(PI)和阻力指数(RI),并计算血管指数(VI)。术后应用抗CD34单克隆抗体对组织进行免疫组化学染色,计数微血管密度(MVD)。结果 三维超声测得的子宫内膜病变的血管参数在良性组与恶性组之间差异有统计学意义。根据ROC曲线,以VI值1.59条/cm3为诊断界点的敏感度和特异度分别为95.00%,81.20%;VI与MVD呈正相关;子宫内膜癌的VI值在Ⅰ期与Ⅱ期、Ⅱ期与晚期差异均有统计学意义。结论 三维能量成像能在术前客观反映子宫内膜病变处的血流情况,可为鉴别良、恶性病变提供有价值的诊断信息。

      Objective To observe difference between benign and malignant endometrial conditions in women with postmenopausal bleeding with three-dimensional color power angiography (3D-CPA). Methods An analysis was performed of 77 women with postmenopausal bleeding. All patients were assessed with 3D-CPA. Resistance Index (RI),pulsatility index (PI) and peak systolic velocity (PSV, cm/s) were recorded automatically, then the 3D power Doppler mode was switched on. Then the QLab program was used to achieve vascularization index (VI). All clinicopathologic data were obtained. All the cervical tissue samples were obtained after operation. MVD of the tissue samples was assessed immunohistochemically (SP method). Results Significant differences of PSV, PI, RI and VI were found between benign and malignant endometrial conditions. PSV, PI, RI and VI in endometrial carcinoma were significantly lower than those in benign endometrial condition. On the basis of the analysis of ROC curve, the cut off values of PI, RI and VI was 0.48, 0.45 and 1.59 counts/cm3, respectively. There was significant positive correlation between MVD and VI (r=0.72). There was no significant correlation in PSV, PI, RI and MVD. Significantly differences of VI and MVD values were noted in Stage Ⅱ tumors compare with Stage Ⅰ tumor and in stage late compared with Stage Ⅱ tumor. Conclusion 3D-CPA can be used to evaluate the information in vascularization objectively, and provide valuable diagnostic information for women with postmenopausal bleeding.
