      目的 通过对冠状动脉狭窄静态模型进行CT扫描,探讨利用不同CT图像后处理技术评估血管狭窄程度的价值。方法 利用绷带、充盈稀释对比剂的盐水袋等模拟肺组织、心腔;使用面粉、黄油、石蜡制作模拟硬化斑块,选取内径为3 mm的塑料管作为模拟冠状动脉,建立冠状动脉狭窄静态模型。对模型进行CT扫描,由3名医师均采用曲面重建(CPR)、多平面重建(MPR)及最大密度投影(MIP)图像评估冠状动脉模型的狭窄程度。CT扫描完成后将模型制成冰冻切片,由1名医师在光镜下测量狭窄程度。结果 模型的CT图像近似于实际冠状动脉狭窄患者的图像。利用同一图像后处理技术评估狭窄程度时,3位医师的一致性较高。利用CPR、MPR评价狭窄程度的正确诊断率与MIP相比,差异均有统计学意义(P均<0.05)。结论 合理地选用各种图像后处理技术综合判断狭窄程度对于提高诊断的准确性十分重要。
英文摘要:
      Objective To observe the value of different post-processing techniques in the diagnosis of vessels stenosis with CT scanning of the static model of coronary artery stenosis. Methods A roll of bandage and a plastic bag filled with diluted contrast medium were used to simulate the lung and the heart. Flour, butter and paraffin were mixed to make the simulative atherosclerosis plaques. Plastic tube with diameter of 3 mm was chosen to simulate the coronary artery. The static model of coronary artery stenosis was established. CT scanning was performed, and the images were reconstructed with curved planar reformation (CPR), multiplanar reformation (MPR) and maximum intensity projection (MIP). The stenosis degree of the simulating vessel was evaluated by 3 radiologists. Then the frozen section of the simulating vessel was observed under microscope by 1 pathologist, while the stenosis degree was measured. Results CT images of the model were similar to those of the patients with coronary artery stenosis. There was high agreement of the diagnostic results between the 3 radiologists. Significant difference was found between the accuracy in evaluating the stenosis degree with observing CPR and MPR images and that of obtained with MIP image (all P<0.05). Conclusion It is extremely important to adopt suitable post-processing technique in improving the diagnostic accuracy of vessel stenosis.
