       目的 观察成人原发性甲状腺功能减退症(简称成人型甲减)患者甲状腺激素(TH)治疗前后局部脑血流(rCBF)改变,探讨成人型甲减致脑中枢神经系统损害的原因。方法 对15名正常对照组及27例成人型甲减患者TH治疗前、后分别进行局部脑血流灌注SPECT显像。结果 27例成人型甲减患者24例出现rCBF降低,其中两侧大脑半球呈弥漫性、斑片样稀疏、缺损改变8例;大脑皮质局部稀疏、缺损改变16例;3例患者脑显像基本正常(包括1例轻型临床甲减和2例亚临床甲减患者)。经过TH治疗后15例患者rCBF基本恢复正常,9例rCBF明显改善。TH治疗后双侧额叶、颞叶及基底节区rCBF有不同程度的改善(P均<0.05),且与正常对照组比较差异无统计学意义(P均>0.05)。结论 成人型甲减患者存在不同程度的rCBF减低,这与其引起的神经精神症状可能密切相关,早期、积极、正规的TH治疗对脑功能的恢复很重要。

      Objective To observe the changes of regional cerebral blood flow (rCBF) perfusion imaging in adult primary hypothyroidism patients pre- and post-thyroid hormone (TH) therapy, and to explore the causes of psychoneural dysfunction in this disease. Methods rCBF were measured by SPECT in 15 normal controls and 27 hypothyroidism patients pre- and post-TH therapy, respectively. Results rCBF of 24 hypothyroidism patients decreased. Diffuse and patching sparseness and defect in bilateral cerebral hemisphere were found in 8 patients. Local sparseness and defect in cerebral hemisphere were detected in 16 patients. rCBF were normal in 3 patients, including 1 mild hypothyroidism and 2 sub-clinical hypothyroidism. After the therapy, rCBF became normal in 15 patients and improved significantly in 9 patients, rCBF in bilateral frontal lobes and temporal lobes improved significantly (all P<0.05), and showed no difference with the controls (all P>0.05). Conclusion The changes of rCBF can be found in adult hypothyroidism patients, which may be related to the neural or psycho symptom in these patients. Early, positive and normalized TH therapy may be important for cerebral function recovery.
