      目的 分析肾透明细胞癌(CCRCC)超声造影特点与病理分期、分级的相关性。方法 回顾性分析50例经病理证实的CCRCC的超声造影表现,包括肿瘤增强均匀性、肿瘤周边高增强环、增强程度和增强模式,并与TNM分期、Fuhrman分级结果进行比较。结果 CCRCC在pT3期相对增强百分数(ΔP%)显著高于pT1期和pT2期(P均<0.05);CCRCC在pT2~3期多表现为不均匀增强(P<0.05);肿瘤周边高增强环多见于pT1期(P<0.01);在增强模式上,CCRCC主要表现为"快进"(94.00%)和"快退"(88.00%),各分期间的差异均无统计学意义(P均>0.05)。以上超声造影特点与Fuhrman分级均未见显著相关性。结论 不均匀增强、缺乏周边高增强环和高ΔP%提示pT3期CCRCC,与之相反提示pT1期CCRCC,pT2期CCRCC则呈不均匀增强、缺乏周边高增强环和相对低的ΔP%。

      Objective To analyze the relationship between contrast-enhanced ultrasound (CEUS) features of clear cell renal cell carcinoma (CCRCC) to TNM stage and Fuhrman grades. Methods CEUS appearance of 50 CCRCCs confirmed pathologically were retrospectively analyzed. For each lesion, the homogeneity of enhancement, the presence of perilesion hyper-enhancement rim, the degree of enhancement and the modality of enhancement were evaluated and compared with the TNM stage and Fuhrman grades. Results The tumor enhancement degree relative to normal renal parenchyma (ΔP%) in pT3 was higher than pT1 and pT2 stages (all P<0.05). CCRCCs tended to be homogeneous enhancement in pT2—3 stage (P<0.05). Each of the patient with lymphatic or distant metastasis had a heterogeneous CCRCC. The detection rate of perilesion hyper-enhancement rim in pT1 stage was significantly higher than that in pT2—3 stage (P<0.01). CCRCCs mainly showed "fast wash-in" (94.00%) and "fast wash-out" (88.00%) modalites, and there was no statistical significance between stages. All these CEUS features had no statistical corelation between Fuhrman grades. Conclusion Heterogeneous enhancement, lack of perilesion hyper-enhancement rim and high ΔP% suggest a pT3 CCRCC, and the opposite features suggesting pT1 CCRCC, while pT2 CCRCC tend to be heterogeneous enhancement, lack of perilesion hyper-enhancement rim and low degree ΔP%.
