       目的 采用波谱学方法无创性研究肝脏脂质沉积相关影响因素及其对获得高分辨可析性波谱的影响。方法 44位正常及弥漫性脂肪肝志愿者均接受磁共振扫描,所获谱线采用SAGE后处理软件分析,对肝脏中含量最高的脂质(亚甲基峰Lip2,1.3 ppm)的含量与受检者一般情况、波谱预扫描结果及肝脏余脂质含量采用等级相关分析;把受检者按肝脏脂质含量Lip2高低分为两组,两组的一般情况、波谱预扫描匀场效果、抑水效果比较采用独立样本的t检验。结果 肝脏Lip2峰下面积与受检者一般情况呈正性相关,与波谱预扫描抑水效果呈负相关;Lip2峰下面积与Lip1峰高、Lip2峰高、Lip3峰高及峰下面积、Lip4峰高及峰下面积均为正相关;肝脏脂质含量较低组较肝脏脂质含量较高组的平均体质量小,平均BMI小,平均年龄小,抑水效果更好。结论 肝脏脂质沉积的增多为年龄、体质量及BMI等相关的体内脂肪沉积增多,BMI为肝脏脂质沉积正相关最为密切的预测因素,抑水为负相关最为密切的预测因素;Lip2含量增多伴随Lip1、Lip3及Lip4含量增多,但相关程度并不一致;肝脏脂质含量增多时影响磁共振波谱预扫描抑水效果,从而更难获得高分辨的可析性波谱。
英文摘要:
      Objective To analyez impact factors of liver lipid levels with in vivo magnetic resonance spectroscopy (MRS). Methods Prospective study of liver 1H-MRS was performed with a GE Signa Excite HD 3.0T system and eight-channel torso phased-array coils using PRESS sequence. Forty-four volunteers were enrolled in this study. All data were processed using Spearman rank correlation analysis and independent samples t-test. Forty-four volunteers, including nonhepatic disease or fatty liver subjects, underwent MR scan. All data were processed with software SAGE. Relationship between Lip2 (methylene peak Lip2, 1.3 ppm) and general condition, spectral shimming and water suppression and other liver lipid contents were analyzed with spearman rank correlation analysis. The subjects were then divided into two groups according to the level of Lip2. Independent samples t-test was used to compare level of Lip2 with general condition, spectral shimming and water suppression between two groups. Results There were positive correlation between the Lip2 peak area and general conditions of subjects, negative correlation between the Lip2 peak area and water suppression. There were positive correlations between the Lip2 peak area and Lip1 peak height, Lip2 peak height, Lip3 peak height and peak area, Lip4 peak height and peak area. The group with lower Lip2 had lower weight, lower BMI, younger age and better water suppression, compared with the group with higher Lip2 level. Conclusion Lipid accumulation in the liver is the result of the increased fat portion of the body depending on age and weight. Body mass index has closest positive relationship, while water suppression has closest negative relationship with liver lipid levels. Lip2 content increases with Lip1, Lip3 and Lip4 content increase, but the correlation is not consistent. The increase of lipid content indicates that it may be harder to achieve effective water suppression.
