目的 对非肿块样强化与肿块样强化乳腺癌的动态增强磁共振成像(DCE-MRI)特征进行比较。 方法 收集269例经穿刺活检证实为乳腺癌,且在化疗前或手术前接受乳腺MRI检查的病例。根据强化类型分为肿块样强化病变组及非肿块样强化病变组,比较两组动态增强参数差异性。 结果 269例乳腺癌患者中,非肿块样强化28例(10.41%),肿块样强化241例(89.59%)。50.00%非肿块样强化乳腺癌增强后晚期达到强化峰值,而81.74%肿块样强化乳腺癌早期即达强化峰值(P<0.001)。39.29%非肿块样强化乳腺癌呈流入型曲线,42.86%呈流出型曲线;75.93%肿块样强化乳腺癌呈流出型时间-信号强度曲线,仅9.13%呈流入型曲线。两种强化类型病变的流出率、信号增强比(SER)亦存在统计学差异。 结论 与肿块样强化乳腺癌比较,非肿块样强化乳腺癌倾向于增强后缓慢强化、呈现流入型时间信号强度曲线。 |

Objective To compare dynamic characteristics of MRI between non-mass-like and mass-like enhancement of breast cancer, in order to confirm the difference between the two enhancement patterns of dynamic contrast-enhanced magnetic resonance imaging (DCE-MRI) in breast cancer. Methods Totally 269 patients with confirmed breast cancer underwent breast MR before treatment, and the dynamic characteristics of MRI were collected. The patients were grouped into the non-mass-like enhancement group and the mass-like enhancement group, and comparisons in dynamic parameters were made between the two groups. Results Totally 28 patients manifested as non-mass-like enhancement (10.41%) and 241 of mass-like enhancement (89.59%). Half of non-mass-like patients achieved peak enhancement in the late period after contrast material was injected, while 81.74% of mass-like enhancements showed this sign in the early period (P<0.001). Significantly larger part of non-mass-like enhancements (39.29%) showed a resistant type curve, and 42.86% in a washout type, while only 9.13% of mass-like enhancements in a resistant type, most of them in a washout type (75.93%). Washout rate and signal enhancement ratio (SER) of non-mass-like enhancements were significantly different from those of mass-like ones. Conclusion Compared with mass-like enhancement, breast cancer of non-mass-like enhancement tends to show a slow enhancement in a resistant type on dynamic MRI. When dynamic characteristics are explored in the researches of MRI with breast cancer, they should be analyzed separately according to different enhancement patterns. |
