目的 探讨双能CT(DECT)对显示手、足肌腱的临床应用价值。方法 对40例患者(手、足肌腱疾病患者28例,手、足肌腱正常患者12例)行双能CT扫描,球管电压分别为140 kV、80 kV,管电流1∶4。将减影后的数据导入后处理工作站行容积重建(VR)、多平面重建(MPR)及曲面重建(CPR),观察肌腱及周围组织情况。所有病例均经手术或结合临床及其他影像学资料综合证实。结果 DECT不仅能够清晰显示大多数正常手、足肌腱的外形、止点;而且能够清晰显示肌腱病变,如纡曲、增粗、粘连、断裂、缺失等,诊断准确率达93.18%。结论 DECT能够准确显示大多数手、足肌腱的正常解剖结构及异常病变,具有快速、无创的优点。 |

Objective To investigate the clinical value of dual energy CT (DECT) in observing tendons of hand and foot. Methods Forty patients, including 28 with tendon traumas in hands or feet and 12 with normal tendons (controls) underwent CT scan in dual energy mode with tube voltage of 140 and 80 kV and a ratio of 1∶4 between tube currents. The reconstructed images were post-processed with volume rendering (VR), multiplanar reconstruction (MPR) and curved planar reconstruction (CPR). Then the insertion, edge, profile, density and continuity of tendons were observed and analyzed. The diagnosis was confirmed with operation, clinical manifestations and other imaging data. Results Almost all normal hand and foot tendons could be displayed clearly. Abnormal findings of tendons, such as circuity, thickening, adhesion, breaking and loss also could be observed clearly. The diagnostic accuracy was 93.18%. Conclusion The anatomy and pathologic changes of hand and foot tendons can be displayed clearly with dual energy CT, which has advantages of rapidity and noninvasiveness. |
