      目的 探讨肝脏局灶性炎性病变的超声造影图像特点。方法 回顾性分析经病理或临床综合诊断的10例肝脏局灶性炎性病变(肝结核4例,细菌性肝脓肿6例)患者的常规灰阶超声及超声造影声像图特征。观察病灶位置、大小、边界、内部回声,重点观察增强模式、增强水平及随时相变化等特点。结果 10例患者共12个肝局灶性炎性病灶。超声造影示其中5个呈周边环状增强伴中央无增强,3个呈弥漫性增强,4个呈蜂窝状增强。动脉相有9个病灶呈高增强且早于周围肝实质,3个病灶与周围肝实质呈同步等增强;门脉相及延迟相,9个病灶呈等增强,3个病灶呈低增强。12个病灶中11个诊断正确,1个病灶误诊,超声造影诊断符合率为91.67%。结论 肝局灶性炎性病灶超声造影具有一定的特征,有助于提高肝脏局灶性炎性病变诊断的准确性。
英文摘要:
      Objective To investigate the perfusion features of hepatic focal inflammatory lesions with contrast-enhanced ultrasound (CEUS). Methods The ultrasonic characteristics of conventional ultrasound and CEUS in 10 patients with hepatic inflammatory lesions (4 with hepatic tuberculosis, 6 with liver abscess) were retrospectively analyzed. All patients were finally proved with pathological and clinical diagnosis. The location, size, boundary and internal echo of lesions were observed, and the enhancement level, enhancement modality and the changes in different phases were observed. Results Among 12 lesions of 10 patients, rim enhancement with central non-enhanced areas was showed in 5 lesions, diffuse enhancement was observed in 3 lesions, and honey comb-like enhancement was found in 4 lesions. In arterial phase, early hyper-enhancement was found in 9 lesions, while simultaneously iso-enhancement was found in the other 3 lesions. In portal and late phase, iso-enhancement was showed in 9 lesions and hypo-enhancement in the others. Of the 12 hepatic inflammatory lesions, CEUS approach correctly diagnosed 11 lesions, misdiagnosed 1 lesion, the diagnostic accuracy was 91.67%. Conclusion The ultrasonic characteristics of CEUS in hepatic focal inflammatory lesions is helpful to improve the diagnostic accuracy of hepatic focal inflammatory lesions.
