目的 探讨高频超声射频数据处理技术(RF-data)评估单纯收缩期高血压(ISH)、糖尿病(DM)、单纯收缩期高血压合并糖尿病(ISH+DM)患者颈总动脉结构和功能改变的应用价值。 方法 应用RF-data技术比较ISH、DM、ISH+DM组患者各23例与正常对照组23名颈总动脉的舒张期内径(Dd)、收缩期内径(Ds)、内中膜厚度(IMT)和颈总动脉扩张性(CD)、单点脉搏波传播速度(PWVβ)、僵硬度(β)的改变。 结果 ①Ds和Dd:ISH组与ISH+DM 组间差异无统计学意义,ISH与ISH+DM两组均明显高于DM组,ISH组、ISH+DM组和DM组三组均高于正常对照组;②IMT:ISH+DM组显著高于ISH组和DM组,DM组显著高于ISH组,ISH组、ISH+DM组和DM组三组均高于正常对照组;③CD: ISH+DM组显著低于ISH组、DM组, ISH组与DM组间差异无统计学意义,ISH组、ISH+ DM组和DM组三组均显著低于正常对照组;④PWVβ、β:ISH+DM组显著高于ISH组、DM组,ISH组与DM组间差异无统计学意义,ISH组、ISH+DM组和DM组三组均显著高于正常对照组。 结论 RF-data能够准确评估ISH、DM、ISH+DM患者颈总动脉结构和功能改变。DM、ISH患者颈总动脉IMT增厚,血管弹性减低;ISH+DM时,血管病变进一步加重。 |

Objective To explore the value of ultrasound radio frequency data technology (RF-data) for evaluating the changes of common carotid arterial structure and function in isolated systolic hypertension (ISH), type 2 diabetes mellitus (DM), isolated systolic hypertension complicated with type 2 diabetes mellitus (ISH+DM) patients. Methods Patients with ISH, DM, ISH+DM and normal subjects (each n=23) were enrolled. With RF-data, the changes of the carotid systolic diameter (Ds), diastolic diameter (Dd), intima-media thickness (IMT), carotid distensibility (CD), local pulse wave velocity (PWVβ) and stiffness (β) were compared among the above groups. Results ①Common carotid arterial Ds, Dd in ISH and ISH+DM group were higher than those in DM group, while no difference was found between ISH and ISH+DM group, while in ISH, DM and ISH+DM group were higher than those in normal control group. ②Common carotid arterial IMT in ISH+DM group was higher than that in DM and ISH group, while in DM group was higher than that in ISH group, in ISH, DM and ISH+DM group were higher than that in normal control group. ③CD in ISH+DM group was lower than that in DM and ISH group, while no significant difference was found between DM and ISH group. CD in ISH, DM and ISH+DM group were lower than that in normal control group. ④PWVβ and β in ISH+DM group were higher than those in DM and ISH group, whereas no significant difference was found between DM and ISH group. PWVβ and β in ISH, DM and ISH+DM group were higher than those in normal control group. Conclusion RF-data can accurately evaluate the changes of common carotid arterial structure and function in ISH, DM, ISH+DM patients. Carotid intima-media thickness increases and arterial elasticity decreases in DM and ISH patients. Serious carotid arterial lesion happens in DM+ISH patients. |
