| 目的 探讨前纵隔淋巴瘤的影像特征。方法 收集21例病理证实的前纵隔肿块的淋巴瘤,回顾性分析其影像学特征。结果 21例影像学均表现为前纵隔巨大肿块的淋巴瘤,男11例,女10例。年龄11~42岁20例,72岁1例。6例HD,其中5例为女性;15例NHL,其中10例为男性。13例肿块向双侧生长,8例肿块偏一侧生长,20例前纵隔内跨区生长,20例病灶累及血管间隙。9例肿块密度均匀,12例见低密度灶,2例见钙化灶。18例巨大肿块周围可见多个软组织结节影,11例肺门、颈部或腋窝淋巴结增大。19例不同程度推移、压迫大血管和心脏,7例上腔静脉受侵,9例左侧头臂静脉受侵。10例合并胸腔积液,6例合并心包积液。1例累及前胸壁。结论 前纵隔淋巴瘤有一定的影像特征,仔细分析大部分能与前纵隔其他恶性肿瘤进行鉴别。 |

| Objective To assess the imaging characteristics of lymphoma of anterior mediastinum. Methods To review the imaging finding of 21 cases with anterior mediastinum lymphoma retrospectively. Results All the patients with lymphoma displayed enormous tumor in anterior mediastinum, 11 male and 10 female. Twenty cases aged from 11 to 42 years old, 1 aged 72 years. There were 5 female in 6 cases of HD and 10 male in 15 NHL. Tumors in 13 cases grown bilateral, 8 cases grown side incline; 20 cases straddled anterior mediastinum and infiltrated the interspace of great vessels. Nine tumors had even density, 12 tumors had low density, 2 tumors had calcification. A total of 18 cases had many swelling lymph nodes around the tumor; 11 cases had swelling lymph nodes in hillum of lung or cervix or axillary; 19 tumors justled and compressed great vessels and heart in different degree; 7 cases invaded superior vena cave; 9 cases invaded brachiocephalic vein; 10 cases had pleural effusion; 6 cases had pericardial effusion and 1 case invaded anterior chest wall. Conclusion Lymphoma of anterior mediastinum has some imaging characteristics and it can be diagnosed correctly by means of analysing the imaging signs carefully. |
