目的 探讨高频超声、CT薄层扫描加增强、99mTc-甲氧基异丁基异晴(MIBI)双时相平面显像三种检查方法对尿毒症继发性甲状旁腺功能亢进症(SHPT)腺体增生病灶的诊断价值。方法 35例尿毒症患者均行高频超声、CT薄层扫描加增强、99mTc-MIBI双时相平面显像核医学检查,将所有增生腺体在B超定位下行细针穿刺活检或手术后病理检查,分别计算三种检查方法诊断敏感性、特异性,以及任意两种方法联合检查及三种方法联合诊断的敏感性、特异性。结果 高频超声、CT薄层扫描加增强、99mTc-MIBI对增生甲状旁腺检测的敏感性和特异性依次为74.07%,95.24%;75.93%,93.18%;90.74%,100%。B超联合CT、B超联合99mTc-MIBI、CT联合99mTc-MIBI、B超联合CT及99mTc-MIBI检查的敏感性和特异性依次为88.89%,96.00%;92.60%,96.15%;98.15%,98.15%;100%,98.18%。结论 99mTc-MIBI双时相平面显像核医学检查对尿毒症继发性甲状旁腺功能亢进患者甲状旁腺的检测敏感性高于CT和B超,三种方法检查的特异性无明显差异,任意两种方法联合应用敏感性和特异性明显提高。 |

Objective To explore the diagnosis value of high-frequency ultrasonography, thin slice CT plain and enhanced scanning and 99mTc-MIBI biphasic scintigraphy in detecting hyperplastic parathyroid glands of uremic patients with secondary hyperparathyroidism (SHPT). Methods High-frequency ultrasonography, thin slice CT plain and enhanced scanning and 99mTc-MIBI biphasic scintigraphy were performed in thirty-five uremic patients to detect the hyperplastic parathyroid glands. Then, biopsies with thin needles were conducted with B ultrasound instruction. The diagnosis sensitivity and specificity of the three techniques, combined application of any two of them and combined application of three methods were computed respectively. Results The sensitivities and specificities of high-frequency ultrasonography, thin slice CT plain and enhanced scanning and 99mTc-MIBI biphasic scintigraphy to diagnose secondary hyperparathyroidism were 74.07%,95.24%; 75.93%,93.18%; 90.74%,100%, respectively; the sensitivity and specificity of combined application of ultrasonography and CT were 88.89% and 96.00%; the sensitivity and specificity of combined application of ultrasonography and 99mTc-MIBI biphasic scintigraphy were 92.60% and 96.15%; the sensitivity and specificity of combined application of CT and 99mTc-MIBI biphasic scintigraphy were 98.15% and 98.15%; the sensitivity and specificity of combined application of three methods were 100% and 98.18%. Conclusion The sensitivity of 99mTc-MIBI biphasic scintigraphy SHPT is higher than that of high-frequency color Doppler ultrasonography and CT, but there are no significant difference in specificity among the three techniques. The sensitivity and specificity in combined application of any two of them are significantly higher than that in using one of them. |
